28例婴幼儿围手术期意外性体温过低的原因分析及护理干预

摘　　要：目的：探讨婴幼儿围手术期意外性体温过低的原因及护理干预。方法：选择2007年1月～2008年12月我院围手术期122例婴幼儿的临床资料,所有患儿在相对湿度40%～60%、室温22～24℃的条件下进行手术,做持续耳膜温度监测,并记录麻醉方式、手术时间、手术部位、全身麻醉者苏醒时间。结果：以体温〈36.5℃为低体温。122例患儿中,术中发生低体温者28例,占22.95%,28例低体温患儿的全身麻醉时间、手术时间比未发生低体温的94例患儿延长,且其全身麻醉患儿苏醒时间也比未发生低体温的患儿延长。结论：全身麻醉、手术时间长、手术创伤大是低体温发生的高危因素。有效的护理干预,如患儿身体保温、适当提高手术室温度、输注保温液体以及器械护士熟练配合降低手术时间是降低婴幼儿围手术期意外性体温过低发生的重要措施。Objective：To explore the cause analysis of 28 cases of hypothermia infant during operation period and its nursing intervention.Methods：122 cases of preoperative patient were enrolled in this study,and the temperature and relative humidity of the operation rooms were kept in 22-24 ℃ and 40%-60%,respectively.Eardrum temperature,anesthesia method,operation time,operative site and awaking time of general anesthesia were recorded in all patients during operation period.Results：28 cases were observed hypothermia during operation period,the incidence of hypothermia was 22.95%（28/122）;the ratios of general anesthesia,long-time operation and thoracic and abdominal surgery in the 28 cases hypothermia infants were significantly higher than those in other 94 cases infants.The mean awaking time of general anesthesia in hypothermia infants was significantly higher than that in normal body temperature infants.Conclusion：General anesthesia,long-time operation and thoracic and abdominal surgery maybe the high risk factors which can induce the hypothermia in the operation course of infant,some effective nursing intervention methods including keeping body warm,increasing properly the operating room temperature,transfusing warm liquid and lowering the operation time by the coordination of tacit agreement between the nurse and doctor,could be taken to lower the incidence rate of hypothermia.
