PPH术治疗急性嵌顿性混合痔疗效及对肛门控便功能的影响

摘　　要：目的探讨采用吻合器痔上黏膜环形切除术(PPH)治疗急性嵌顿性混合痔(AIIH)的临床疗效及对肛门控便功能的影响。方法采用PPH手术治疗病例78例,分为嵌顿痔组和混合痔组,对术后疗效及并发症进行分析。结果术后两组疗效及控便机能的变化无明显差异;嵌顿痔术前术后直肠静息压、直肠排便收缩压、肛管静息压的变化有显著性差异(P<0.05),肛管最大收缩压变化无明显差异(P>0.05);术后肛管直肠测压两组无显著性差异(P>0.05)。结论嵌顿痔是痔病理发展的严重阶段,PPH手术适用急性嵌顿混合痔。Objective To investigate the efficacy and bowel control of procedure for prolapsed hemorrhoids（PPH）with embolectomy surgery for acute incarceration of circumferential mixed hemorrhoids（AIIH）.Methods The specimens of 78 cases underwent PPH from May 2007 to May 2008 were collected.They were divided into AIIH group and mixed hemorrhoids group.Symptom relief,complications,and patient satisfaction were analysized.Results There were no difference in the quantity,the variety and the depth of inflammatory cell infiltration between them（P0.05）.No different in assessment of the symptom relief,complications,patient satisfaction,and patients＇ perception of bowel function were found in the two groups after surgery.No difference in the rectal pressure of AIIH was found either before and after surgery or between the two groups after surgery.Conclusion The pathological change of AIIH group is similar to that of mixed hemorrhoids group.Acute incarceration of internal hemorrhoids is the serious pathological stage of hemorrhoids;it isn＇t cause by bacterial infection.The PPH surgery is a good choice for acute incarceration of circumferential mixed hemorrhoids.
