祛毒活血法结合西药治疗脓毒症的临床研究

摘　　要：目的观察祛毒活血中药配合西医干预治疗脓毒症的临床疗效。方法将30例脓毒症患者随机分为治疗组(予西医常规疗法结合祛毒活血方)和对照组(采用西医常规疗法治疗),每组各15例,疗程为7天。观察治疗前后中医证候、APACHEⅡ评分、T淋巴细胞亚群以及TNF-α、IL-1、IL-6和IL-10的变化情况。结果治疗组、对照组总有效率分别为80.0%、55.3%,两组疗效差异有统计学意义(P<0.05)。治疗后2组中医证候积分和APACHEⅡ评分值均下降(P<0.05),且治疗组明显低于对照组同期水平(P<0.05);治疗后治疗组TNF-α、IL-1、IL-6和IL-10水平下降(P<0.05),且低于同期对照组(P<0.05)。结论祛毒活血中药配合西医常规疗法治疗脓毒症可有效改善患者临床症状,减轻脓毒症的炎症反应。Objective To observe the therapeutic effects of combined stasis-dissolving and blood-activating therapy and western medicine in treating sepsis.Methods Thirty cases of sepsis were randomly divided into treatment group and control group,with 15 cases in each group.The treatment group was treated with combined stasis-dissolving and blood-activating therapy and western medicine,and the control group with western medicine.After seven-day treatment,the clinical symptoms and signs,APACHEⅡ,levels of lymphocyte subsets and cytokine were observed in both groups before and after treatment.Results After treatment,the total effective rate was 80.0% in the treatment group and 55.3% in the control group,with a difference between them（P〈0.05）;TCM symptoms score and APACHEⅡ score were decreased in both groups（P〈0.05）,with better effects in the treatment group than in the control group（P〈0.05）;the serum levels of TNF-α,IL-1,IL-6 and IL-10 were decreased in the treatment group（P〈0.05）,and decreased insignificantly in the control group（P〉0.05）,with more significant decrease in the treatment group than in the control group（P〈0.05）.Conclusion Combined stasis-dissolving and blood-activating therapy and western medicine can improve the clinical symptoms and relieve inflammation in the treatment of sepsis.
