加替沙星为基础的三联疗法治疗幽门螺杆菌感染的临床疗效分析

摘　　要：目的:分析加替沙星为基础的三联疗法治疗幽门螺杆菌(Hp)感染的临床疗效,探讨更佳的根除方案。方法:将180例患者随机分为3组。治疗(A组)60例:加替沙星400 mg,甲硝唑0.4 g,2次/d,兰索拉唑30 mg,2次/d;治疗(B组)60例:加替沙星200 mg,2次/d,甲硝唑0.4 g,2次/d,兰索拉唑30mg,2次/d;对照组(C组)60例:克拉霉素0.5 g,2次/d,甲硝唑0.4 g,2次/d,兰索拉唑30 mg,2次/d。疗程均为1周。停药大于4周后复查胃镜。结果:A组、B组、C组Hp根除率分别为92%、90%、87%,差异无统计学意义(P>0.05)。不良反应总发生率为25%,A组与C组间有较显著差异(P<0.05),B组与C组间有非常显著差异(P<0.01)。A组、B组、C组的治疗费用分别为310.15元、309.41元、379.46元。结论:加替沙星为基础的三联疗法治疗Hp感染疗效确切,且具有安全、费用低的优势,值得推广应用。Objective： To analyze the effects of gatifloxacin-based triple therapy on Helicobacter pylori（Hp） infection in order to find out better treatment.Methods： A total of 180 patients were randomly divided into 3 groups.group A（60 cases） was given gatifloxacin 400mg ＋ metronidazole 0.4g,twice per day,and lansoprazole 30 mg,twice per day;group B（60 cases）,gatifloxacin 200 mg,twice per day＋ metronidazole 0.4 g,twice per day,lansoprazole 30 mg,twice per day;group C served as control group（60 cases） and was given clarithromycin 0.5g,twice per day,metronidazole 0.4 g,twice per day,lansoprazole 30 mg,twice per day.All groups were treated for a treatment course of l week.4 weeks after drug withdrawal,all patients were rechecked under endoscopy.Results： Hp eradication rates were 92%,90% and 87% for group A,B,C;respectively showing significant differences among each other（P0.05 or P0.01）.Expenditure for group A,B and C were 310.15 yuan,309.41 yuan and 379.46 yuan.Conclusions： Gatifloxacin-based triple therapy is safe,cost-effective for Hp infection,and it′s worth for application in large scale.
