全麻、硬膜外复合麻醉在老年上腹部手术患者中的应用

摘　　要：目的:观察全麻、硬膜外复合麻醉在老年上腹部手术患者中的临床效果,探讨其可行性及安全性。方法:将82例老年腹部手术患者随机分为全麻联合硬膜外麻醉组(GEA组)和单一全麻组(GA组),GEA组给予全麻联合硬膜外麻醉,GA组给予单一全麻,观察两组麻醉效果及临床各项指标,并进行对比分析。结果:GEA组在手术后拔管时间、完全恢复自主呼吸时间及完全清醒时间均较GA组有明显提前,而GEA组全麻时麻醉药物的使用量均明显少于GA组,组间比较差异均有统计学意义(P<0.05);GEA组手术过程中MBP、HR、SpO2较平稳,GA组MBP、HR波动较大。结论:全麻、硬膜外复合麻醉具有麻醉效果好,药物用量小,术后恢复快等特点,是临床老年腹部手术患者麻醉方式的最佳选择。Objective： To observe the clinical effects of general anesthesia and epidural anesthesia on abdominal surgery for elder patients,and to explore its feasibility and safety.Methods： A total of 82 elderly patients with abdominal surgery were randomly divided into general anesthesia combined with epidural anesthesia group（GEA group） and a single anesthesia group（GA group）.Patients in GEA group were given general anesthesia combined with epidural anesthesia,while patients in GA group were given only general anesthesia.The clinical effect and various indexes were observed and compared.Results： The extubation time after surgery,full recovery time of spontaneous breathing and awake time of the GEA Group were shorter than those of the GA group,while the dosage of narcotic drugs during general anesthesia of the GEA group was significantly less than that of the GA group,with significant differences in those indexes between two groups（P0.05）.The MBP,HR and SpO2 were relatively stable throughout the surgery in the GEA group,while the MBP and HR were unstable in the GA group.Conclusions： General anesthesia combined with epidural anesthesia is effective,with small drug dosage and quick recovery after surgery.It is preferred choice in clinical anesthesia for abdominal surgery.
