长方案注射HCG日血清雌二醇/孕酮比值对体外受精-胚胎移植结局的影响

摘　　要：目的研究长方案促排卵患者注射人绒毛膜促性腺激素(Human chorionic gonadotropin,HCG)日血清雌二醇(Estradiol,E2)/孕酮(Progesterone,P)比值(E2/P)对体外受精-胚胎移植(in vitrofertilization and embryo transfer,IVF-ET)结局的影响。方法回顾性分析2010年413个IVF-ET周期的资料,将E2/P值分为3组,Ⅰ组:E2/P≤1.2,Ⅱ组:1.2<E2/P≤5.0,Ⅲ组E2/P>5.0,对3组的相关数据进行分析。结果 3组获卵数、2PN受精率均存在差异,有统计学意义(P<0.05)。卵巢过度刺激综合征发生率Ⅲ组高于Ⅰ、Ⅱ组,Ⅲ组与Ⅱ组之间比较有统计学意义(P<0.05)。种植率及妊娠率Ⅱ组最高而流产率最低,流产率Ⅱ组低于Ⅲ组,有统计学意义(P<0.05);种植率、妊娠率Ⅰ组均低于Ⅱ、Ⅲ组,有统计学意义(P<0.05)。结论合适的E2/P值有利于获得较高的妊娠率、更低的流产率和卵巢过度刺激综合征发生率,并使IVF-ET的结局更好。Objective To explore the influence of serum estradiol and progesterone ratio（E2/P）on the day of human chorionic gonadotropin（HCG） on the outcome of cases received long protocol in vitro fertilization and embryo transfer（IVF-ET）.Methods The data of 413 cycles undergoing IVF-ET treatment from January 2008 to December 2008 were analyzed retrospectively.Cases were divided into 3 groups（as Group I： E2/P≤1.2,GroupⅡ：1.2E2/P≤5.0,Group Ⅲ： E2/P5.0） according to the serum estradiol and progesterone ratio（E2/P）on the day of human chorionic gonadotropin（HCG）,relative data were compared among 3 groups.Results The number of oocytes and fertilization rate of the 3 groups were significant differences（P0.05）.The rate of severe ovary hyperstimulation syndrome（OHSS） was higher in Group Ⅲ than that in Group I and Group Ⅱ,the difference between group Ⅲ and Group Ⅱ was significant（P0.05）.The implantation rate and pregnancy rate of Group Ⅱ was the highest while abortion rate was the lowest.Moreover,the abortion rate of Group Ⅱ was lower than that of group Ⅲ（P0.05）.The implantation rate and pregnancy rate in Group I were lower than those in group Ⅲ and Group Ⅱ（P0.05）.Conclusion Appropriate ratio of E2/P on the day of HCG may be benefit the outcome of IVF-ET and achieve higher pregnancy rate,lower abortion rate and lower incidence rate of OHSS.
