中青年颈椎病患者的X线摄片特点

摘　　要：目的对40岁以下拍摄X线颈椎正、侧位片患者的影像学资料进行分析,旨在探讨中青年颈椎病患者的X线摄片特点。方法 2008年5月—2010年5月间共有1925例15～40岁(中位年龄为33.2岁)的患者行颈椎X线正、侧位片检查,对其中548例伴有上肢牵拉放射疼痛、手指发麻等感觉异常者加拍双斜位片,217例患者行CT或MRI检查。结果颈椎正常生理弧度改变1260例,占65.45%;颈椎骨质增生1481例,占76.94%;颈椎椎间隙狭窄541例,占28.10%;颈椎椎间孔狭窄476例,占24.72%;颈椎滑移26例,占1.35%;项韧带钙化194例,占10.08%。结论早期中青年颈椎病变以颈椎生理弧度异常改变及颈椎轻度骨质增生为主。普通颈椎X线正、侧位摄片对早期颈椎病的发现及诊断有重要作用。Objective To retrospectively analyze the cervical X-ray films of anteroposterior and lateral positions of young patients （≤40 years old） with cervical lesions in order to investigate the early film features of the disease. Methods Totally one thousand nine hundred and twenty-five patients aged between 15-40 years （median age 33.2 years） received cervical spinal radiography during May 2008 to May 2010 in our department. Among them, there were 548 undergoing spine dual oblique filming, and 217 undergoing CT and MRI scanning due to radiating pain to upper limbs, tingling fingers and other abnormalities. Results Abnormal physiological changes in cervical curvature were found in 1 260 cases, accounting for 65.45% of the total. There were 1 481 cases with cervical bone hyperplasia, accounting for 76.94%, 541 cases with intervertebral cervical stenosis, accounting for 28.10%, 476 cases with cervical foraminal stenosis, accounting for 24.72%, 26 cases with cervical slip, accounting for 1.35%, and 194 cases with nuchal ligament calcification, accounting for 10.08%.Conclusion Cervical abnormalities in young patients with early cervical lesions are mainly abnormal cervical curvature and mild cervical bone hyperplasia. Cervical spine radiography at anteroposterior and lateral positions play an important role in early detection and diagnosis of cervical disease.
