无创双水平正压通气在COPD合并呼吸衰竭中的应用观察

摘　　要：目的探讨无创双水平正压通气在COPD合并呼吸衰竭中的应用。方法 2008年2月—2010年9月收治的280例COPD合并呼吸衰竭患者,随机分为常规治疗组142例,无创正压通气组138例,观察两组临床效果。结果两组治疗前血PH值、PaO2、PaCO2比较无显著差异性(P>0.05)。两组治疗24h后、治疗72h后血PH值、PaO2、PaCO2比较有显著差异性(P<0.05)。两组气管插管率;两组病死率、平均住院日比较有显著差异性(P<0.05)。结论无创双水平正压通气在COPD合并呼吸衰竭治疗中临床疗效确切,改善患者临床症状,改善肺通气和肺换气,可以明显降低病死率。Objective To investigate the application of non-invasive bi-level positive pressure ventilation in COPD patients with respiratory failure.Methods In February 2008-Sep 2010 280 cases admited COPD with respiratory failure were randomly divided into the conventional treatment group,142 cases of noninvasive positive pressure ventilation group 138 cases and clinical effects were observed.Results Before treatment,the blood PH value,PaO2,PaCO2 comparison no significant difference（P0.05）.two groups after 24h,73h post-treatment blood PH value,PaO2 PaCO2 comparison significant difference（P0.05）.Group rate of endotraccheal intubation;group mortality,average length of stay comparison significant difference（P0.05）.Conclusions The non-invasive bi-level positive pressure ventilation in COPD patients with respiratory failure have definity effect,improve the clinical symptoms,improve pulmonary ventilation and lung ventilation,can significantly reduce mortality.
