第三代喉罩在较长时间腹腔镜胆囊手术中使用的安全性探讨

摘　　要：目的：观察第三代喉罩（Proseal Laryngead mask airway PLMA）加静脉吸入复合全麻在手术时间较长（超过120min）的腹腔镜胆囊切除手术中使用的安全性.方法：收集我院2008年5月至2010年10月间使用三代喉罩行腹腔镜胆囊切除术手术时间大于2小时的患者80例,随机选择其中40例于喉罩通气120分钟后改为气管插管通气（TT组）,另外40例持续喉罩通气至手术结束（PLMA组）,对比分析插喉罩组和气管插管组患者全麻诱导前,麻醉诱导后2小时以及拔管或拔除喉罩时HR、MAP、PETCO2的变化,术中、术后发生各种并发症的例数,记录两组的手术、麻醉时间.结果：两组患者各指标没有差异.结论：第三代喉罩加静脉吸入复合全麻在时间较长（超过120min）的腹腔镜胆囊切除术中使用是安全的,而且具有操作简单、循环稳定、并发症少的优点,值得临床推广.Aim at the third generation throat （proseal laryngead a mask airway plma） with the inhale composite all linen in the operation hours longer than 120min ） （ the peritoneal cavity mirror gallbladder big use of security.
