经皮肾镜碎石术的并发症及其防治

摘　　要：目的回顾经皮肾镜治疗肾结石的并发症,评价其临床安全性及有效性。方法我科从2008年10月—2010年5月通过经皮肾镜治疗肾结石86例,男50例,女36例,年龄27~81岁,其中铸型结石5例,重复肾上半肾结石1例,孤立肾结石2例。结果术后大出血2例,腹腔积液2例,术后发热3例,腹腔积液患者1例行剖腹探查,1例行腹腔置管引流,其余均经非手术治疗治愈。结论开展经皮肾镜取石术（PCNL）初期最为常见的并发症是出血、感染及周围脏器损伤。Objective To evaluate the clinical safety and efficiency by review the complications of percutaneous nephrostolithotomy at initial stage.Methods The data of 86 cases of PCNL from October 2008 to May 2010 were reviewed,including 50 male cases and 36 female cases,their age range from 27 to 81 years.Of them,5 cases had staghorn nephrolithiasis,2 cases of solitary kidney calculi,1 case with renal duplication.Results Complications occurred in 7 cases（8.1%）,2 cases had massive hemorrhage,2 cases had hydroperitoneum,3 cases of fever attacks（T38.5℃）,All patients were cured by conservative treatment except one patient.Conclusion At the initial stage of PCNL,hemorrhage,febricity and injury of peripheral viscera are common.
