胸腹水与血液中生化指标联合检测对良恶性胸腹水鉴别的诊断价值

摘　　要：目的探讨患者胸腹水中的乳酸脱氢酶(LDH)、总蛋白(TP)、糖(GLU)和血清中该三项指标联合检测对良、恶性胸腹水鉴别的临床意义。方法将85例患者的胸腹水分为两组,良性胸腹水组46例,恶性胸、腹水组39例,分别对两组胸、腹水中的LDH、TP和GLU及两组患者相对应血清中的LDH、TP和GLU进行测定,并对检测结果进行比较分析。结果两组患者胸、腹水中LDH的浓度和TP的浓度有统计学差异(P<0.05);两组患者胸、腹水中GLU的浓度无统计学差异(P>0.05);两组患者胸、腹水中LDH与血清中LDH浓度比值、胸、腹水中TP与血清中TP浓度比值和胸、腹水中GLU与血清中GLU浓度比值均有统计学差异(P<0.05)。结论胸、腹水中LDH、TP和GLU与血清中LDH、TP和GLU联合测定对鉴定胸、腹水的性质具有重要的诊断价值。Objective To evaluate the clinical value of combined detection of the low-dose heparin（LDH）,total protein（TP）,glucose（GLU） in the blood serum and pleuroperitoneal fluids in the diagnosis of malignan and benign pleuroperitoneal fluids.Methods 85 patients with pleuroperitoneal fluids were divided into two groups,46 cases of benign pleuroperitoneal fluids group and 39 cases malignant pleuroperitoneal fluids group.Then,we tested the content of LDH、TP、GLU in the blood serum and pleuroperitoneal fluids in all the patients and compared the results.Results The concentration of LDH and TP in pleuroperitoneal fluids has statistical differences in the two groups（P0.05）.Whereas,there is no statistical differences in terms of GLU in pleuroperitoneal fluids in the two groups（P0.05）.The ratio of LDH in the blood serum and pleuroperitoneal fluids in the two groups has statistical differences（P0.05）.Similar results are found in the ratios of TP and GLU in the two groups.Conclusion The combined detection of LDH,TP and GLU in the blood and pleuroperitoneal fluids has special clinic value in the diagnosis of malignant and benign pleuroperitoneal fluids.
