摘  要:目的:了解我院鲍曼不动杆菌对常用抗菌药物的耐药现状及标本分布,为临床治疗提供依据。方法:采用回顾性方法统计分析186株鲍曼不动杆菌的标本来源、感染科室分布及耐药情况。结果:痰液标本中鲍曼不动杆菌对各类常用抗菌药物均表现严重的耐药现象,对亚胺培南的耐药率最低,为35.6%;其次为哌拉西林/他唑巴坦,耐药率为45.2%;再次为氟喹诺酮类如环丙沙星、左氧氟沙星,耐药率超过50.0%;对不含酶抑制剂的三代头孢如头孢曲松、头孢他啶的耐药率均达到了77.0%以上;而对阿米卡星、庆大霉素等普通氨基苷类的耐药率则超过了80.0%。结论:该临床分离的鲍曼不动杆菌耐药严重,且呈多重耐药,因此临床必须重视合理使用抗菌药物,以减少或延缓多重耐药菌株的产生。Objective: To investigate the distribution and situation of antimicrobial resistance of Acinetobacter baumannii isolated from our hospital,so as to provide reference for antimicrobial treatment.Methods: A total of 186 strains of Acinetobacter baumannii were retrospectively analyzed according to the source of specimens,infections in different departments and drug-resistance.Results: The drug resistance of Acinetobacter baumannii was serious.The lowest resistance rate was to imipenem,followed by piperacillin/tazobactam,and then it was to be ciprofloxacin and levofloxacin that belonged to fluoroquinolones(FQNS).The resistance rate of Acinetobacter baumannii to them was 35.6%,45.2% and 50.0%,respectively.However,the resistance rate of Acinetobacter baumannii to the third generation cephalosporins that not contained enz catastaltic,such as ceftriaxone,ceftazidime and so on,was as high as 77.0%.At the meanwhile,the most critical one was aminoglycosides(ie.gentamicin and amikacin),the resistance rate of Acinetobacter baumannii was more than 80%.Conclusion: Acinetobacter baumannii isolated from the patients shows high rate of resistance to routine antimicrobial agents,and even has presented multiple antibacterial resistance.So we must keep an eye on the nosocomial infection and rationally use the antimicrobial agents in order to decrease or delay the production of multi-drug resistant strains.
