带腓肠神经营养血管蒂逆行岛状皮瓣在小腿下1/3及足踝部软组织缺损的临床应用

摘　　要：目的：探讨带腓肠神经营养血管蒂逆行岛状皮瓣修复小腿下1/3及足踝部皮肤软组织缺损的治疗效果。方法：采用带腓肠神经营养血管蒂逆行岛状皮瓣修复小腿下1/3及足踝部皮肤软组织缺损22例的临床实践,证实该皮瓣的解剖位置及适用范围。结果：22例皮瓣均成活,术后随访1~36个月,外形满意,皮瓣功能恢复良好。结论：带腓肠神经营养血管蒂逆行岛状皮瓣是修复小腿下1/3及足踝部皮肤软组织缺损的可行方法。Objective To investigate the effect of soft tissue defect repairing of lower third of the leg and foot defect by reversed island flap pedicled with nutrient vessels of sural nerve.Method Total 22 patients with soft tissue defects around the heel,malleolus,achilles tendon and the low 1/3 part of anterior tibial were reconstructed by use of reversed sural neurovascular fasciocutaneous island flap.Results All flaps were survived.The function and shape of recipient and donor site were satisfied.Conclusion The reversed island flap pedicled with nutrient vessels of sural nerve which has reliable blood supply,long-pedicled,simple operative technique,not sacrificed major vessel is ideal donor site for soft tissue defect repairing of lower third of leg and foot defect.
