不同浓度布比卡因蛛网膜下腔阻滞效果观察

摘　　要：目的探讨不同浓度布比卡因在蛛网膜下腔的阻滞效果。方法选择60例膝关节镜手术患者,随机分为3组,选用腰硬联合阻滞麻醉方式。Ⅰ组蛛网膜下腔使用0.25%布比卡因4m l;Ⅱ组蛛网膜下腔使用0.33%布比卡因3m l;Ⅲ组蛛网膜下腔使用0.5%布比卡因2m。l麻醉过程中连续观察患者生命体征和无痛平面变化情况。结果所有患者顺利完成手术,麻醉镇痛效果好,生命体征无明显变化,患者麻醉时间、无痛平面和阻滞时间在同组间无明显差异,而3个小组之间有明显的差异。结论相同剂量的布比卡因,在蛛网膜下腔阻滞时,阻滞平面随布比卡因浓度的改变而变化。Objective To compare the spinal anesthesia effects of different concentrations of bupiva- caine. Methods A double blind, randomised study was performed on 60 knee arthroscopy patients who aged 18-45 years, were divided into three groups averagely（ I , II andⅢ ）. Spinal anesthesia was carried out with 0. 25% ,0.33% and 0.5% bupivaeaine in three different groups. The cardiovascular changes and spinal anesthetic block height of patients in the study were traced. Results A11 patients safely finished the surgery with acceptable cardiovascular changes （ P 〉 0.05 ）. Onset time and duration of blockade were significant difference between the three groups （ P 〈 0.05 ）, but insignificant difference within each group （ P 〉 0.05 ）. Conclusion At same dosage, the concentration of bupivacaine can alter spinal anesthetic block height.
