摘  要:目的探讨痰浊内阻型原发性高血压与混沌理论的相关性。方法选择2008年3月至2011年3月本院心血管内科及中医病房住院的痰浊内阻型原发性高血压2~3级患者120例(男72例,女48例);同期本院体检者113例(男57例,女56例)作为健康对照组。参考杓状曲线和散点图采用动态血压监测(ABPM)技术中的24h收缩压平均值(24hSBP)、24h舒张压平均值(24hDBP)、24h收缩压标准差(24hSBPSD)、24h舒张压标准差(24hDBPSD)、夜间血压下降率、24h收缩压变异系数(24hSBPCV)、24h舒张压变异系数(24hDBPCV)进行分析,观察两组变化。结果痰浊内阻型原发性高血压组24hSBP、24hDBP、24hSBPSD、24hDBPSD均明显高于健康对照组〈24hSBP(ramHg,1mmHg=0.133kPa):145.50±9.90比110.60±8.20,24hDBP(mmHg):85.62±11.08比66.68±6.78,24hSBPSD(mmHg):15.66±4.52比11.95士3.14,24hDBPSD(mmHg):11.32土2.89比9.28±2.03,均P〈0.053,夜间血压下降率明显低于健康对照组(4.2%(5/120)比96.5%(109/113),P〈0.053,且男性、女性患者与相应健康人群比较差异亦均有统计学意义(均P〈0.05);而24hSBPCV、24hDBPCV均无明显差异(均P〉0.05)。结论痰浊内阻型原发性高血压与混沌理论存在相关性。Objective To study the relativity between phlegm blocking type (痰浊内阻型) essential hypertension and chaos theory. Methods One hundred and twenty cases (male 72, female 48) with phlegm blocking type essential hypertension grade 2 - 3 admitted to Departments of Cardiovascular Medicine and Traditional Chinese Medicine (TCM) of Wuhan Hospital of TCM and Western Medicine from March 2008 to March 2011 were enrolled; while 113 healthy individuals (male 57, female 56) served as the healthy control group. According to dipper curve as well as scatter gram, the ambulatory blood pressure monitoring (ABPM) technology was used to analyze the changes of the means of 24 hours systolic blood pressure (24 h SBP) and 24 hours diastolic blood pressure (24 h DBP), 24 hours systolic blood pressure standard deviation (24 h SBPSD), 24 hours diastolic blood pressure standard deviation (24 h DBPSD), night blood pressure fall rate, coefficient of variation of 24 hours systolic blood pressure (24 h SBPCV) and coefficient of variation of 24 hours diastolic blood pressure (24 h DBPCV) were observed in two groups. Results In the group of phlegm blocking type essential hypertension, the 24 h SBP, 24 h DBP, 24 h SBPSD and 24 h DBPSD were significantly higher than those in the healthy control group (24 h SBP (mm Hg, 1 mm Hg= 0. 133 kPa).. 145.50i9.90 vs. 110.60±8.20, 24 h DBP (ram Hg).. 85.62±11.08 vs. 66.68±6.78, 24 h SBPSD (mmHg):15.66±4.52 vs. 11.95±3.14, 24 h DBPSD (mm Hg): 11.32±2.89 vs. 9.28±2,03, all P〈0. 05], and the night blood pressure fall rate was obviously lower than that in the healthy control group (4.2% (5/120) vs. 96.5% (109/113), P〈0. 053 ; and there was significant difference statistically between male patients and healthy male individuals as well as between female patients and healthy female individuals (both P〈0. 05) ; whereas there was no significant difference in 24 h SBPCV and 24 h DBPCV (all P〈0.05).
