家庭访视频率在糖尿病健康教育管理模式中的应用研究

摘　　要：目的研究不同频率家庭访视的健康教育模式对糖尿病患者依从性、空腹血糖、餐后2 h血糖、糖化血红蛋白水平等各项指标的影响。方法将150例糖尿病患者随机分为4组,对实验组采用不同频率家庭访视模式和传统模式进行糖尿病健康教育。入组时和3月后分别对各组的患者采用糖尿病患者知识量表、自我管理量表对患者糖尿病知识和自我管理水平进行评价,同时监测空腹血糖、餐后2 h血糖、糖化血红蛋白水平。结果与对照组比较,实验组1和实验组2在家庭访视后3个月,空腹血糖的降低和糖尿病知识水平及自我管理水平差异有统计学意义,餐后2 h血糖、糖化血红蛋白降低的水平,3组差异均有统计学意义。结论最终采用2次/周的家庭访视方法,持续3个月,以后1次/月,对糖尿病患者提高自我管理水平和控制血糖水平有重要意义,并能达到效果和资源的有效整合。Objective To explore the influence of different home visit frequency for health education models on patients′ compliance,fasting blood glucose,two-hour postprandial blood glucose,glycated hemoglobin levels,diabetesrelated knowledge and other indicators.Methods 150 cases of diabetes were randomly divided into experimental group 1,group 2,group 3 and control group.We used different home visit frequency model and traditional model for diabetes health education,and evaluate the fasting glucose,two-hour postprandial blood glucose,glycated hemoglobin levels of each group when they were first grouped and data were collected 3 months later.At the same time,diabetes knowledge scale and self-management scale were also recorded.Results Compared with the control group,the reduced level of fasting glucose,the increased diabetes knowledge and self-management after 3 months home visit of experimental group 1 and group 2 were statistically significant.The lower of two-hour postprandial blood glucose,glycated hemoglobin levels of experimental group 1,group 2 and group 3 were statistically significant.Conclusion Based on the findings of research,conclusion can be drawn that 1 every 2 weeks of home visits by lasting three months is advisable and then once a month health education for diabetic patients can effectively achieve the goal of resources combination.
