微波治疗25例慢性肢体淋巴水肿的组织学和酶细胞化学研究

摘  要:对25 例淋巴水肿患者进行微波治疗,治疗前后用光镜、普通电镜及电镜酶细胞化学观察病变组织的变化。发现治疗前:⑴表皮增厚;⑵组织间隙大量蛋白和水分;⑶血管旁和组织间隙有大量的细胞浸润;⑷丰富且活跃的纤维母细胞;⑸稠密的胶原纤维束;⑹巨噬细胞不活跃,CMP酶染色少。治疗后:⑴皮肤组织炎性病变减轻;⑵毛细血管呈扩张状;⑶组织间隙水分和蛋白大量减少;⑷纤维母细胞活性下降;⑸巨噬细胞活跃,CMP酶染色增强增多。说明微波通过调节蛋白分解细胞( 巨噬细胞) 的活性,增强毛细血管的重吸收功能,以及促进组织内炎性病变的消退,达到改善水肿的目的。The Plastic and Reconstructive Department,Shanghai Second Medical University,Shanghai 200011$$$$To explore the mechanism of microwave action in treatment of chronic limb lymphedema,twenty five patients with chronic limb lympledema were treated by using microwave (2450 MHz,100W) 1h/d for 40day.The pathologic changes of limb skin tissue were observed with light-,electron-microscope and enzymic cytochemical stains before and after treatment of microware.The results were as follows:Before the treatment:⑴Cuticular layer of the skin was thickened;⑵There were much more water and proteins in the intercellalarspace of the skin tissue;⑶A lot of inflammatory cells invaded around blood vessels and in the intercellularspace of the skin tissues;⑷Many active fibroblasts were seen;⑸Observed collagen fibres aggregation;⑹Macrophages activity were decreased and CMP enzyme was less stained.After treatment:⑴The skin inflammation became thin;⑵Dilation of skin capillaries;⑶Water and proteins in the intercellularspace of the skin tissue were much reduced;⑷Fibroblasts activity was decreased in the skin tissue;⑸Macrophages have shown strongly activity and staining of CMP enzyme was enhanced. The findings indicated that the microwave treatment could achieve the aim of improving lymphedema by modulating the activity of macrophages,enhancing reabsorption of the tissue fluid by capillaries and diminishing inflammation in tissue of chronic lambs lymphedema.
