摘 要:目的探讨单纯后路松解原位弯棒技术对重度僵硬的非角状脊柱侧凸的治疗效果。方法采用后路松解、原位弯棒技术治疗包括各种病因导致的重度僵硬的非角状脊柱侧凸畸形14例,依据患者的影像学资料、并发症的发生情况等评估临床效果。结果脊柱侧凸的主弯平均Cobb角为86.3°(70~97°),柔韧性为21.3%(8%~28%),术后矫正率为61.7%(48.5%~74.3%),手术前的冠状面失衡约22 mm,术后为9 mm,手术无严重并发症发生。结论单纯后路松解可以增加脊柱侧凸僵硬节段的柔韧性,原位弯棒技术治疗非角状重度脊柱侧凸的近期疗效满意,与去旋转等技术相结合,可以减少并发症的发生。Objective To study the effect of large and rigid scoliosis with posterior spinal release and in situ rod bending technique.Methods A total of 14 consecutive patients undergone posterior spinal release and in situ rod bending technique for the management of large and rigid scoliosis of any etiology and followed up at least 16 months were included.The radiographic studies,complications were used to assess outcomes.Results The primary curve Cobb angle correction were from 86.3°to 32.7°,the correction rate was 61.7%.Coronal imbalance was 22 mm before and 9 mm after surgery.No major complication occurred in this group of patients.Conclusion The posterior spinal release alone can increase the flexibility of the rigid scoliosis.In situ rod bending technique is an effective procedure for the management of large and rigid scoliosis.
