山东部分社区盆底功能障碍性疾病的流行病学研究

摘　　要：目的调查山东省部分社区人群盆底功能障碍性疾病（PFD）的流行病学特征、患病相关因素以及对生活质量影响的情况。方法采用问卷调查和常规体检方式,根据国际尿控学会的诊断标准,对山东省部分社区妇女进行健康查体时获得临床资料进行分析。结果共调查2 354人,有效问卷2 326份,年龄跨度为1970岁,平均年龄（39.54±15.13）岁,PFD的患病率为25.4%（591/2 326）,其中压力性尿失禁（SUI）和盆腔脏器脱垂（POP）的患病率分别为12.3%（286/2 326）、13.1%（305/2 326）,SUI和POP的患病率随着年龄的升高呈升高的趋势（P=0.001）。调查显示,年龄、便秘、子宫切除术后、阴道分娩、产次、产程延长及会阴裂伤是SUI发生的相关因素;而年龄、便秘、子宫切除术后、阴道分娩、产次、产程延长、绝经是POP发生的相关因素。PFD对生活质量影响的严重程度和平均年龄无关,重度生活质量受损的患者就业率下降。结论 PFD的发病和年龄、便秘、子宫切除术后、阴道分娩、产次、产程延长有关,对患者生活质量有严重损害。Objective To investigate the prevalence and risk factors of pelvic floor dysfunction（PFD）and its influence on quality of life.Methods A questionnaire survey and physical examination were conducted in a community of Shandong Province,China.Results 2 354 people were investigated and 2 326 questionnaires were effective.The average age was（39.54±15.13）years（range,19-70）.The incidence of PFD was 25.4%（591/2 326）： 12.3%（286/2 326） for stress urinary incontinence（SUI） and 13.1%（305/2 326） for pelvic organ prolapse（POP）.Incidences of SUI and POP were positively related to patients′ ages（P=0.001）.Data analysis showed that age,conatipation,hysterectomy,vaginal delivery,labor times,prolonged labor and perineum rupture were influencing factors for SUI.While age,conatipation,hysterectomy,vaginal delivery,labour times,prolonged labour and menopause were influencing factors for POP.The average age had no relevance with living quality of PFD patients,while severely impaired quality of life significantly decreased the employment rate.Conclusion Pelvic floor disorders are associated with age,constipation,hysterectomy,vaginal delivery,parity,prolonged labor,perineal laceration and menopause,and significantly impair patients′ quality of life.
