摘  要:目的了解30周龄以上早产儿甲状腺功能状态,并探讨其临床意义。方法选取2008年8月至2010年6月出生的42例胎龄为33~36周的早产儿(早产儿组)及15例足月健康适龄儿,采集静脉血,分离血清,运用电化学发光法行甲状腺激素、甲状腺球蛋白及其相关抗体的测定,包括TT3、TT4、FT3、FT4、TSH、Tg、Tg-Ab。结果早产儿组血清TT3、TT4、FT3、FT4、Tg、Tg-Ab明显低于对照组,差异有统计学意义(P均<0.05);2组间血清TSH浓度差异无统计学意义(P>0.05)。结论由于受多种因素的影响,30周龄以上早产儿甲状腺功能存在暂时性低下现象,应当及时给予合理的甲状腺素替代治疗,治疗的剂量和疗程需要进一步的研究探讨。Objective To evaluate the thyroid function of preterm infants born at more than 30 weeks’ gestation and explore its clinical significance.Methods A total of 42 preterm infants born at 33-36 weeks’ gestation (neonate group) and 15 full-term healthy infants (control group) born in our hospital between August 2006 and June 2008 were enrolled in this study.The blood of femoral vein was taken within 24 hours after birth.Serum total triiodothyronine(TT3),total thyroxine (TT4),free triiodothyronine (FT3),free thyroxine (FT4),thyrotropin (TSH),thyroglobulin (Tg) and thyroglobulin antibody (Tg-Ab) were detected by electrochemiluminescence method.Results The levels of TT3,TT4,FT3,FT4,Tg,Tg-Ab in neonate group were obviously lower than those in control group (all P 〈 0.05),while no significant difference was found in the levels of TSH between two groups (P 〉 0.05).Conclusions Due to influences of many factors,the phenomenon of transient thyroid hypofunction exists in premature infants born at more than 30 weeks’ gestation.The reasonable replacement therapy of thyroxine in these infants should be timely given,and the dosage and the course of treatment should be further explored.
