摘 要:目的通过对尿L-FABP、血肌酐(Scr)、肾小球滤过率(GFR)等临床参数的测定和评价,探索尿L-FABP与慢性肾脏病的相关性。方法实验选取无肝脏损害慢性肾脏病组(CKD组)60例,CKD1-2期20例、CKD3-4期20例、CKD5期20例,健康组20例。留取研究对象的血液及尿液标本,检测血肌酐(Scr)、肾小球滤过率(GFR)及尿L-FABP水平。结果 Wilconxon秩和检验显示CKD各分期间尿L-FABP水平比较差异有统计学意义(H=69.05,P<0.001),亚组分析显示CKD1-2期尿L-FABP水平高于健康组,CKD3-4期尿L-FABP水平高于CKD1-2期,CKD5期尿L-FABP水平高于CKD3-4期。Pearson相关性分析显示尿L-FABP水平与GFR呈显著负相关(rs=-0.942,P<0.001),与Scr呈显著正相关(rs=0.948,P<0.001)。结论慢性肾脏病时尿L-FABP水平显著升高,随着CKD分期的进展,尿L-FABP水平逐渐上升。尿L-FABP与GFR呈显著负相关,与Scr呈显著正相关。尿L-FABP可以作为协助诊断慢性肾脏病的临床生化指标。Objective To investigate the relationship between L-FABP and CKD,We assessed the clinical parameters of urinary L-FABP,serum Scratinine and glomerular filtration rate.Methods Five groups were involved in this experiment:without liver injury CKD group(n=60) and healthy group(n=20),CKD1-2 stage group(n=20),CKD3-4 stage group(n=20),CKD5 stage group(n=20).To obtain serum and urinary samples of studied objects,the level of urinary L-FABP,serum Scratinine and glomerular filtration rate were measured by ELISA and biochemical methods respectively.Results Rank and inspection shows CKD Wilconxon comparison of the urinary L-FABP levels during statistically significant differences(H=69.05,P0.001),subgroup analysis showed CKD1-2 stage urinary L-FABP levels higher than health group,CKD3-4 stage urinary L-FABP levels higher than the CKD1-2 stage,CKD5 stage urinary L-FABP level higher than the CKD3-4 stage.Pearson correlation analysis shows urinary L-FABP levels and GFR is a significant negative correlation(rs=-0.942,P0.001),and Scr are markedly positive(rs=0.948,P0.001).Conclusion Urinary L-FABP levels increased significantly when chronic kidney disease,with CKD stage of development,urinary L-FABP levels gradually rise.urinary L-FABP levels and GFR is a significant negative correlation,associated with Scr is significantly positive.As a clinical biomarker,urinary L-FABP can help to diagnose chronic kidney disease.
