摘 要:目的探讨中国苏皖地区汉族人群C反应蛋白(CRP)基因启动子T-757C多态性分布及其与急性冠脉综合征(ACS)的相关性。方法采用聚合酶链反应-限制性片段长度多态性方法,检测468例ACS患者(ACS组)和282例非冠心病者(对照组)的CRP基因T-757C多态性。结果与对照组比较,ACS组TC基因型频率明显增高(40.4%VS31.2%,P=0.012)。ACS组的TT和CC基因型和T等位基因频率分布与对照组比较差异无统计学意义(分别为56.6%vs63.5%、3.0%VS5.3%和76.8%VS79.1%,P值分别为0.064、0.120和0.308)。在调整年龄、吸烟、收缩压、舒张压、血糖、甘油三脂、低密度脂蛋白胆固醇和高密度脂蛋白胆固醇等危险因素,并经多元Logistic回归分析显示,CRP基因T-757C多态性TC基因型与ACS的发生呈显著相关性(OR=1.439,95%CI1.025~2.019,P=0.036)。结论CRP基因启动子T-757C多态性与ACS密切相关.提示该多态性在中国苏皖地区汉族人群ACS的发病机制中具有一定作用。Objective To investigate the distribution of C -reactive protein (CRP)gene T- 757C polymorphism in the promoter region and the association of the CRP gene T- 757C polymorphisrn with acute coronary syndrome (ACS) in the Chinese Han population of Suwan region. Methods This study was conducted with a case- control design including 468 patients with ACS( ACS group)and 282 control subjects who were free from coronary artery disease(control group). The CRP gent T- 757C polymorphism was determined by polymerase chain reaction and restriction fragment length polymorphism analysis. Results Compared with those in the control group, the frequency of the TC genotype was obviously higher in ACS group (40.4% vs 31.2% ,P = 0.012 ). There was no statistical difference in the frequencies of TT and CC genotype (56. 6% vs 63.5% and 3.0% vs 5.3% , respectively)between patients with ACS and controls (P = 0. 064 and 0. 120, respectively). The prevalence of T allele was also not significantly different between ACS group and control group (76. 8% vs 79. 1% , P = 0. 308 ). According to multivariate Logistic regression analysis, traditional ACS risk factors such as age, smoking, systolic blood pressure, diastolic blood pressure, blood glucose, triglycerides and lower- density lipoprotein cholesterol and high - density lipoprotein cholesterol after the adjustment indicated that there was still significant association of the TC genotype with ACS ( OR = 1. 439,95% CI 1. 025 - 2. 019, P = 0, 036). Conclusion There is significant association of the CRP gene T- 757C polymorphism in the promoter region with ACS, which suggests that CRP gene T - 757C polymorphism plays a role in the development of ACS in the Chinese Han population of Suwan region.
