摘  要:目的探讨复制缺陷型腺病毒介导的血管内皮生长因子(VEGFl65)和血红素氧化酶(HO—1)联合应用,对提高大鼠超长随意皮瓣成活面积的影响和作用机制。方法将25只健康纯种成年雌性SD大鼠的背部两侧各设计-6cmx1cm大小随意皮瓣。皮瓣远端皮下分别注射日的基因联合应用组(Ad—VEGFl65+Ad—HO-1)、基因对照组(Ad—VEGFl65)、基因对照组(Ad—HO—1)、绿色荧光蛋白基因对照组(Ad-11B—GFP)、病毒保存液(Ad—buffer)为空白对照组,共5组。术后7d,分别测量皮瓣的成活面积、HE染色及目的蛋白和CD31免疫组化表达情况及皮瓣新生傲血管密度(MVD)检测等。结果目的基因联合应用组皮瓣成活面积、目的蛋白表达及MVD与其他4组比较,其差异有显著统计学意义(P〈0.01)。结论以复制缺陷型腺病毒为载体,联合应用VEGF和HO-1基因治疗能显著增加大鼠随意皮瓣的成活面积,促进皮瓣存活。Objective To investigate the effect of replication defective adenovirus mediated vascular endothelial growth factor (Ad-VEGF165) and heine oxygeuase-1 ( Ad-HO-1 ) on increasing the survival area of the rat's overlong random flap and its mechanism. Methods Twenty-five female Sprague Dawley Rats were used for this experiment. Each random flap (6 cmx 1 cm) was designed on each side of the ratg back (2 flaps for each rat). Then flaps were divided randomly into 5 groups equally( n = 5 rat/10 flap/group) : The Ad-VEGF165 + Ad-HO-1 (A group) as the experimental group, Ad-VEGF165 ( B group), Ad-HO-I ( C group) and Add 1B-GFP (D group) as the geue control groups, Ad-buffer (E group) as the blank group were injected subcutaneously into the distal end of the flaps. The measurement of the survival area of the flaps, HE staining the expression of interest protein and CD31 detected by IHC and the detection of neoformative microvessel density (MVD) were performed at 7 days. Results Compared with the control groups, the difference of survival area, expression of the interest protein and MVD counting in the experimental group has striking statistical significance ( P〈0.01 ). Conclusion The replication deficient adeuovirus as vector, combined application of Ad-VEGFI65 and Ad-HO-1 can increase obviously the survival area of remote random flap.
