新生儿缺氧缺血性脑病血清MMP-9、IL-6的测定及临床意义

摘　　要：目的探讨新生儿缺氧缺血性脑病(hypoxic ischemic encephalopathy,HIE)患儿血清中基质金属蛋白酶-9(MMP-9)、白细胞介素-6(IL-6)水平的动态变化与脑损伤的关系。方法选择我院收治的HIE 40例(HIE组)采用酶联免疫吸附(ELISA)法测定发病后1 d、3 d、7 d血清MMP-9、IL-6浓度;另采用同种方法测定我院产科同期足月健康新生儿20例(健康对照组)的MMP-9、IL-6水平,并进行比较。结果 HIE组发病后1 d、3 d、7 d血清MMP-9、IL-6水平均较健康对照组升高,差异有统计学意义(P<0.05),以发病后3 d升高最为明显,此后呈降低趋势;对患儿的病情严重程度与发病后3 d血清MMP-9、IL-6水平进行Spesrman等级相关分析,结果显示二者呈显著正相关。结论血清MMP-9、IL-6水平可能对HIE的病情严重程度和预后判断有一定预示作用。Objective To investigate the dynamic changes of serum matrix metalloproteinase-9（MMP-9）,interleukin-6（IL-6） in neonates with hypoxic ischemic encephalopathy（HIE） and its correlation with brain injuries.Methods 40 HIE neonates were chosen as HIE group,the concentrations of MMP-9 and IL-6 were measured by ELISA method at day 1,3,and 7 after invasion.Meanwhile 20 healthy newborns were chosen as healthy control group,and concentration of MMP-9 and IL-6 in both groups were compared.Results 1 d,3 d,7 d after invasion,the levels of MMP-9 and IL-6 in HIE group were significantly higher than that in healthy control group,and the difference was significant（P0.05）,especially on day 3,but both decreased afterwards.Spesrman rank correlation analysis results showed significant positive correlation between the disease degree and MMP-9,IL-6 levels on day 3.Conclusion Serum MMP-9,IL-6 levels may be valuable in predicting the prognosis and degree of disease.
