32例皮肤恶性肿瘤术后缺损巨大双叶皮瓣Ⅰ期修复患者的护理

摘　　要：总结了32例巨大双叶皮瓣Ⅰ期修复皮肤恶性肿瘤术后缺损的护理经验。认为术前做好心理疏导、皮肤护理、督促戒烟;术后监测生命体征,妥善摆放体位,密切观察皮瓣的颜色、温度、肿胀程度和毛细血管充盈反应,早期发现皮瓣血液循环障碍并及时处理。住院期间有4例皮瓣出现血液循环障碍,其中3例经有效的治疗及护理,恢复良好,1例A瓣出现皮瓣尖端局部坏死,经局部换药冲洗1个月后愈合。This paper summarizes the nursing care of 32 patients with skin defects resulted from resection of skin cancer and one-stage repair with bilobed skin flap.Before surgery,nursing care focused on psychological guidance,skin care,and urging to quit smoking were carried out.After surgery,the key points in nursing were monitoring vital signs,position care,close observation of the color,temperature,swelling and capillaries filling conditions of the skin flaps to detect flap ischemia.As a result,flap ischemia occurred in four cases.Three cases recovered after 3 to 5 days of treatment and nursing,one case developed ischemic necrosis in the tip of the skin flap and healed one month later.
