摘　　要：本研究通过检测系统性红斑狼疮(SLE)患者血清中抗巨核细胞抗体,以探讨SLE患者发生血小板减少的发病机制。应用间接免疫荧光法检测36例SLE伴血小板减少患者中的血清抗巨核细胞抗体,并与30例血小板正常的SLE患者和30名正常人对照。结果表明,血清抗巨核细胞抗体阳性在36例SLE伴血小板减少患者中占7例(19.4%),在30例血小板计数正常的SLE患者中有2例(6.7%),及在正常人中有1例(3.3%),后者与SLE伴患者血小板减少患者相比,差异有统计学意义p=0.046),但伴有或不伴有血小板减少的SLE患者之间差异无统计学意义(p=0.132)。结论 :SLE患者血清存在抗巨核细胞的自身抗体,并与部分患者发生血小板减少有关,应扩大病例数作出肯定结论 ,进一步了解SLE所致血小板减少的机理。This study was purposed to investigate the mechanism of thrombocytopenia in patients with systemic lupus erythematosus （SLE） through detecting arlti-megakaryocyte antibodies in SLE patients. The serum anti-megakaryocyte antibodies in 36 SLE cases with thrombocytopenia were detected by using indirect immunofluorescence, the detected rerults were compared with detected results of 30 SLE cases without thrombocytopenia and 30 healthy persons. The rusults showed that the positive incidences of anti-megakaryocyte antibody in serum of 36 SLE cases with thrombocytopenia, 30 SLE cases without thromocytopenia and 30 healthy persons were 19.4% （7/36）, 6.7% （2/30） and 3.3% （1/30） respectively. As compared with SLE patients without thrombocytopenia and healthy persons, SLE patients with thrombocytopenia had higher incidence of anti-megakaryocyte antibodies, moreover there was significant difference between SLE patients with thrombocytopenia and healthy persons （p 〈 0.05 ）, while there was no significant difference between SLE patients with or without thrombocytopenia （ p 〉 0. 05 ）. It is concluded that autoantibodies against megakaryocytes exist in SLE patients and may partially contribute to the incidence of thrombocytopenia in SLE patients. The detection of anti-megakaryocyte antibodies with a enough case number is needed to make a final conclusion on thrombocytopenia pathogenesis in SLE.
