抗环瓜氨酸肽抗体在系统性红斑狼疮关节炎中意义

摘　　要：目的：检测抗环瓜氨酸肽（CCP）抗体在系统性红斑狼疮（SLE）患者中的水平,探索抗CCP抗体与SLE患者关节炎及骨侵蚀的关系。方法：采用酶联免疫吸附试验（ELISA）方法检测141例SLE患者血清中抗CCP抗体水平,并与SLE患者关节临床表现及其他实验室指标进行相关性分析。结果：抗CCP抗体在SLE患者中的阳性率为20.6%（29/141）。其中,在SLE关节炎组抗CCP抗体阳性率为26.7%（20/75）,非关节炎组抗CCP抗体阳性率为13.6%（9/66）,两者间比较相近（P=0.056）,但是SLE关节炎组抗CCP抗体中位水平为16 RU/mL（5~1600 RU/mL）高于非关节炎组的12 RU/mL（4~51 RU/mL）（P=0.004）。抗CCP抗体阳性SLE患者中关节炎组抗CCP抗体中位水平为304.5 RU/mL（28~1600 RU/mL）,也明显高于非关节炎组的30.0 RU/mL（28~51 RU/mL）（P=0.001）。在75例SLE有关节炎表现的患者中,有6例患者出现侵蚀性关节炎,其中5例检测出高滴度抗CCP抗体[459 RU/mL（76~605 RU/mL）]。结论：在SLE中,20.6%的患者体内存在抗CCP抗体,对于有关节炎表现的SLE患者伴有高滴度的抗CCP抗体,可能会发生侵蚀性关节炎,抗CCP抗体对判断SLE关节炎具有重要意义。AIM： To detect the level of anti-cyclic citrullinated peptides（anti-CCP） antibodies in systemic lupus erythematosus（SLE） patients with arthritis,and explore the relationship of anti-CCP antibodies with erosive arthritis in SLE patients. METHODS： The levels of anti-CCP antibodies in 141 case patients with SLE were detected by ELISA.The dependablity between anti-CCP antibodies and clinical features as well as laboratory parameters in these SLE patients were analyzed.RESULTS：The positive rate of anti-CCP in SLE patients was 20.6%（29/141）.The positive rate of anti-CCP in SLE patients with arthritis was 26.7%（20/75）,which in SLE patients without arthritis was 13.6%（9/66）,there was no significant difference（P=0.056）.But the level of in anti-CCP in SLE patients with arthritis（16 RU/mL） was higher than that in SLE patients without arthritis（12 RU/mL,P=0.004）.In anti-CCP antibodies positive SLE patients,the level in arthritis patients（304.5 RU/mL）was higher than that in without arthritis patients（30.0 RU/mL）（P=0.001）.There were 6 SLE patients with erosive arthritis in all subjects and high level anti-CCP antibodies（459 RU/mL） were detected in 5 patients.CONCLUSION： Anti-CCP antibodies can be found in 20.6% of patients with SLE,the patients who had high level of anti-CCP antibodies might initiate erosive arthritis.Anti-CCP antibodies is important to identify SLE patients with arthritis.
