突出物内外靶位注射胶原酶治疗腰椎间盘突出症的临床研究

摘　　要：目的:观察突出物内外靶位注射胶原酶治疗腰椎间盘突出症的临床疗效。方法:筛选确诊为腰椎间盘突出症的患者120例。根据入院先后分为两组。经骶管裂孔穿刺硬膜外前间隙置管法(以下简称对照组)组60例;突出物内外靶位注射胶原酶(以下简称实验组)组60例。临床疗效采用VAS评分和改良MacNab评分,分别评价术后7天,电话随访3、6个月疗效。结果:术后7 d、3、6个月的有效率分别为74%、83%、86.6%;86%、92%、95%,组间比较差异显著性(P<0.05)。结论:突出物内外靶位注射胶原酶治疗腰椎间盘突出症是治疗椎间盘突出症的有效方法,效果明显优于经骶管裂孔穿刺硬膜外前间隙置管法。Objective：To observe the clinical effects of collagenase injected by projection targets of inside and outside prominence for lumbar disc herniation patients.Methods：The 120 diagnosed patients of lumbar disc herniation had been selected to divided 2 groups according to admission.One group（hereinafter referred to as the control group） of 60 cases was treated by epidural space puncture catheter act through sacral hiatus.The other group（hereinafter referred to as the control group） of 60 cases was treated by collagenase injected by projection targets of inside and outside prominence.The clinical ecffects were assessed by VAS scores and modified MacNab score in seventh day after surgery.The treatment effects of third and sixth month were evaluated by elephone follow-up.Results：The efficients of control group in seventh day,third and sixth month after surgery were respectively 74%、83%、86.6%;The efficients of experimental group in seventh day,third and sixth month after surgery were respectively 86%、92%、95%.The difference between the control group and experimrntal group was significant（P0.05）.Conclusions：The collagenase injected by inside and outside projection targets was an effective treatment method for lumbar disc herniation.The effects were obviously superior to treatment by simple epidural space puncture catheter act through sacral hiatu.
