生长抑素治疗食管胃底静脉曲张破裂出血的护理

摘　　要：目的探讨生长抑素治疗食管胃底静脉曲张破裂出血的效果与护理。方法将32例食管胃底静脉曲张破裂出血的病人按随机数字表法分为观察组和对照组,每组16例。所有病人均给予补液、扩容治疗。在此基础上,对照组应用垂体后叶素50 U加入5%葡萄糖注射液250 mL中以25滴.min-1的滴速输入。观察组应用生长抑素3 mg加入0.9%氯化钠注射液250 mL中静脉滴注,首剂静脉推注5 mL,后以20.8 mL.h-1滴速输液泵维持。2组均用药3 d为1个疗程。在治疗的同时加强心理、饮食等方面的护理。对2组病人用药后的疗效及不良反应进行比较。结果观察组总有效率(93.75%)明显高于对照组(68.8%),不良反应发生率明显低于对照组(均P<0.05)。结论合理应用生长抑素,正确实施护理,能收到理想的止血效果。Objective To investigate the effects of Somatostatin and nursing on esophageal and fundus ventriculus variceal bleeding.Methods Thirty-two patients with esophageal and fundus ventriculus variceal bleeding were randomly divided into two groups,with 16 patients in each group.All patients were given fluid replacement and volume expansion.In the control group,50 U Hypophysin and 250 mL 5% glucose were intravenously infused at a rate of 25 drops per minute.In the observation group,3 mg Somatostatin and 250 mL normal solution were administered to patients who received the solution by intravenous bolus injection（5 mL） firstly,and then by intravenous infusion（20.8 mL·h-1） with the infusion pump.Patients were treated for 3 days as a course of treatment.At the same time,psychological and diet nursing care was strengthened.The therapeutic efficacy and adverse reactions were compared between the two groups.Results Compared with the control group,the total effective rate significantly increased and the incidence of adverse reactions obviously decreased in the observation group（all P〈0.05）.Conclusion The rational application of Somatostatin and good nursing care have a good hemostatic effect in patients with esophageal and fundus ventriculus variceal bleeding.
