母牛分枝杆菌菌苗在结核性胸膜炎治疗中的作用

摘　　要：目的评价母牛分枝杆菌菌苗(微卡)胸腔内注射治疗结核性胸膜炎(tubercu lous pleurisy,TP)的效果。方法随机将80例TP患者分为两组:治疗组(40例)和对照组(40例)。两组均采用统一抗结核化疗方案2HRZE(S)/4HR及胸穿抽液,此外治疗组胸腔内注射微卡每周1次,共2次。观察两组病例胸水吸收及胸膜肥厚情况,同时观察微卡副作用。结果治疗组治愈+显效率为95%,对照组为70%,两组对比有显著性差异(P<0.01);治疗组胸膜肥厚率为5.0%,对照组为22.5%,两组对比有显著性差异(P<0.05);治疗组平均胸水吸收时间为19.2 d,对照组为27.9 d,两组对比差异显著(P<0.01);微卡胸腔注射后主要副作用为低热,发生2例。结论微卡能明显加快胸水吸收,降低胸膜肥厚率,用药安全,是治疗TP一种较好的药物。Objective To evaluate the therapeutic effect of intrapleural injection of Mycobacterium vaccae（M.vaccae） vaccine on the treatment of tuberculous pleurisy.Methods Eighty tuberculouis pleurisy patients were randomly divided into two groups：study group（n=40） and controls group（n=40）.All the patients of both groups were treated using chemotherapy with a regimen of 2HRZE（S）/4HR and aspirating pleural effusion.For the study group,the patients were also given intrapleural injections of M.vaccae vaccine once a week for two times.The rates of pleural thickening and absorption of pleural effusion were observed.And at the same time side effects were observed.Results The rates of recovery and improvement were 95% in the study group and 70% in the control group.There was a significant difference between the two groups（P〈0.01）.The rates of pleural thickening were 5.0% in the study group and 22.5% in the control group.There was a significant difference between the two groups（P〈0.05）.The mean days of absorption of pleural effusion was 19.2 for the study group and 27.9 for the control group.There was a significant difference between the two groups（P〈0.01）.The side effects of M.vaccae vaccine were slight fever（2 case）.Conclusion M.vaccae vaccine is an effective and safe drug to reduce pleural thickening in treating patients with tuberculous pleurisy.
