摘 要:目的探讨肾缺血再灌注损伤后肾叶间动脉血流动力学和肾组织转化生长因子-β1(TGF-β1)表达的关系。方法建立兔肾缺血再灌注损伤模型,随机分为假手术组和缺血再灌注组。应用彩色多普勒血流显像(CDFI)监测各组肾叶间动脉的血流动力学变化,免疫组化法检测各组同一时间点肾组织中TGF-β1蛋白的表达水平,并分析其相关性。结果假手术组和缺血再灌注2 h亚组无明显血流动力学改变;缺血再灌注8 h亚组叶间动脉阻力指数(RI)增大,24 h亚组叶间动脉收缩期峰值流速(Vmax)、博动指数(PI)及RI增大。假手术组各亚组TGF-β1表达无明显改变;缺血再灌注各亚组TGF-β1表达随着缺血再灌注损伤的加重而逐渐增强。假手术组各亚组肾组织病理切片未见异常,肾单位清晰可见,排列整齐。缺血再灌注组随着再灌注时间的延长,肾小管上皮细胞越肿胀,再灌注24 h时可见上皮细胞呈气球样改变。假手术组各亚组肾叶间动脉与肾组织TGF-β1表达无相关性(P>0.05)。缺血再灌注组肾叶间动脉的Vmax、PI及RI与肾组织TGF-β1表达存在显著正相关性(P<0.05)。结论肾叶间动脉血流动力学改变与肾组织损伤程度存在高度一致性,CDFI实时检测肾叶间动脉可以有效评价缺血再灌损伤程度。Objective To investigate the relationship between the rabbits renal interlobar arteria hemodynamics and expression of endogenous transforming growth factor-β1( TGF -β1) in renal ischemia - reperfusion injury ( IRI ). Methods Rabbits kidney IRI model was established, 36 rabbits were divide into sham -operated group and IRI groups. Renal interlobar arteria hemodynamic changes in two groups were monitored by color Doppler flow imaging (CDFI), and the TGF-β1 protein expression level of renal tissue was detected by Strept Avidin - Biotin Complex (SABC) method, then the correlation was analyzed. Results There were no significant hemodynamic changes between sham - operated group and IRI 2h subgroups group, RI of renal interlobar artery was increased in IRI 8h subgroup, and Vmax, PI, RI were increased in IRI 24 h subgroup. There was no significant change of TGF- β1 expression in sham -operation group,The TGF-β1 in IRI subgroup was increased with the degree of IRI. In sham - operation group, the disposition of nephrons was clear and regular, while in IRI group, the renal tubular epithelial cells became more and more turgid with the increase of reperfusion time, ballooning degeneration was found after 24 h. There was no correlation of TGF -β1 expression and hemodynamics of interlobar artery in sham - operated group (P 〉 0.05) , while there was obvious positive correlation of Vmax, PI, RI and TGF- β1 expression of renal tissue in IRI group(P 〈 0.05 ). Conclusion There is high uniformity between the changes of renal interlobar artery hemodynamics and the degree of renal tissue injury, CDFI can evaluate renal IRI effectively.
