超声二维应变成像对代谢综合征患者左室收缩功能评价分析

摘　　要：目的探讨超声二维应变成像评价代谢综合征(metabolic syndrome,MS)患者左心室收缩功能的临床价值.方法对代谢综合征不伴左室肥厚(MS-NLVH)患者24例,26例伴左室肥厚(MS-LVH)患者和28例正常人(对照组).采集心尖左室长轴、心尖四腔观和心尖二腔观的二维灰阶动态图像,应用自动功能成像软件自动测量左室长轴方向18节段收缩应变,心尖左室长轴观总应变(GLPS-LAX)、四腔观总应变(GLPS-A4C)、二腔观总应变(GLPS-A2C)及左室长轴平均全局应变(GLPS-Avg).结果两组代谢综合征患者的左心室长轴基底段、中间段、心尖段收缩期纵向峰值应变平均值及左室全局应变值明显低于对照组(P<0.01),而代谢综合征合并左心室肥厚组较非肥厚组降低(P<0.01,或P<0.05).结论超声二维应变成像能评价代谢综合征患者左室收缩功能的早期变化.Objective To assess left ventricular systolic function in patients with metabolic syndrome by two-dimensional strain echocardiography.Methods 24 patients with MS and non-LVH（left ventricular hypertrophy,MS-NLVH group）,26 patients with MS and LVH（MS-LVH group）and 28 healthy subjects（control group）were included.The two-dimensional loop-cinec was obtained in apical 4-chamber view,apical 2-chamber view and long axis of left ventricle（LV）view.Eighteen segments of LV longitudinal peak systolic strain and global peak systolic strain of each view were measured by automated functional imaging（AFI）software of 2DSE.Results The average peak systolic longitudinal strain of different segments and the peak longitudinal global strain in MS-NLVH group and MS-LVH group were significantly reduced when compared with those in control group（P〈0.01）,and these parameters in MS-LVH group were significantly lower than those in MS-NLVH group（P〈0.01,or P〈0.05）.Conclusion Two-dimensional strain echocardiography may be used to assess the LV systolic function in patients with metabolic syndrome.
