艾滋病合并结核病90例患者治疗管理模式的分析

摘　　要：目的:探讨艾滋病合并结核病患者高效抗反转录病毒治疗(HAART)和抗结核治疗时机及实施管理后的治疗效果。方法:成立院内艾滋病、结核病治疗专家小组,在结核病患者中检测HIV。在艾滋病患者中筛查结核,通过X线胸片、痰液检测、组织细胞学检测等筛查结核(TB)/HIV双重感染者,入选者实施抗结核及适时抗病毒治疗,随访1年。结果:在90例TB/HIV双重感染者中,30例结核杆菌阳性28例结核临床治愈,1例丢失,1例死亡。60例结核杆菌阴性中58例结核临床治愈,2例治疗8个月后痰结核菌涂片阳性。两组患者治疗前后自身CD4T淋巴细胞比较差异有统计学意义(P<0.01);组间比较差异无统计学意义(P>0.05)。结论:早发现、早治疗结核,适时抗病毒治疗,有效督导管理能提高患者治疗依从性和结核治愈率,改善患者生命质量。Objective：To investigate the timing of highly active antiretroviral therapy（HAART） and antituberculosis treatment and the therapeutic effects after implementing management in acquired immunodeficiency syndrome（AIDS） complicating tuberculosis（TB）.Methods：To set up the hospital AIDS/TB treatment expert panel.To detect human immunodeficiency virus（HIV） among TB patients and TB among HIV patients.To screen the cases of HIV/TB dual infection by X-ray chest radiograph,sputum test and histocytology detection.The selected cases were given antituberculosis and timely antiviral treatment and followed up for one year.Results：Of 90 cases of HIV/TB dual infection,30 cases were positive Mycobecterium,among them 28 cases of tuberculosis obtained clinical cure,1 case lost and 1 case died;in 60 cases of negative Mycobacterium,58 cases of tuberculosis obtained clinical cure and 2 cases had positive sputum smear after 8 month treatment.CD4 T-cell count showed statistical difference before and after treatment in each group（P0.01）,but which had no statistical difference between 2 groups（P0.05）.Conclusion：Early detection and early treatment of TB,timely ART and effective supervision and management can improve the patient compliance with therapy,raise the cure rate of TB and improve patients quality of life.
