2型糖尿病踝肱指数及相关因素分析

摘　　要：目的:探讨踝肱指数(ankle brachial index,ABI)检查在2型糖尿病患者下肢外周血管病变(peripherals arterial disease,PAD)的患病率及其危险因素。方法:采用血管多普勒诊断仪对151例2型糖尿病患者进行ABI测定,ABI<0.9为合并下肢血管病变组(PAD组),ABI≥0.9为正常组(非PAD组)。比较两组患者的年龄、病程、体质量指数、血糖、HBA1c、血脂。通过统计学方法探讨影响ABI的相关因素。结果:151例患者中检出ABI<0.9者38例,占25.2%,与非PAD组比较,PAD组患者年龄大、病程长、餐后2小时血糖、糖化血红蛋白、胆固醇、低密度脂蛋均明显升高,差异有显著性(P<0.05)。结论:ABI可用于早期诊断糖尿病下肢血管病变;年龄大、病程长、餐后高血糖、糖化血红蛋白高、甘油三酯高、低密度脂蛋白高是PAD的主要危险因素。因此,常规进行ABI检测可早期发现糖尿病PAD,对糖尿患者应更好地控制血糖、血脂。Objective：To investigate the role of detecting ankle brachial index（ABI） in the prevalence of lower limbs peripheral arterial disease（PAD） in type 2 diabetes and to analyze the roles of risk factors.Methods：151 cases of type 2 diabetes were detected ABI with Doppler diagnostic instrument of blood vessels.The patients were divided into PAD group（ABI0.9） and non-PAD group（ABI≥0.9）.Their age,diabetes duration,body mass index（BMI）,fasting plasma glucose and 2 h postprandial plasma glucose（2hPG）,glycohemoglobin（HbA1c）,triglycerides（TC）,cholesterol（C）,high density lipoprotein（HDL） and low density lipoprotein（LDL） were recorded and detected for comparison between the two groups.SPPS11.0 was employed for screening the related factors influencing ABI.Results：38 cases（25.2%） of abnormal ABI（ABI0.9） were detected.PAD group showed older age,longer diabetes duration,higher 2hPG levels and higher levels of HbA1c,TC and LDL-C.The differences were considered significant compared with the non-PAD group（P0.05）.Conclusion：Detection of ABI could be used to the early diagnosis of PAD in diabetes.The main risk factors for PAD are older age,longer diabetes duration,hyperglycemia and hyperlipidemia.The routine determination of ABI may be thereby necessary to early discover diabetic PAD.The diabetic patients should better control blood glucose and blood lipid.
