超声引导下ENCOR真空旋切装置在乳腺微创外科中的应用

摘　　要：目的探讨安珂(ENCOR)真空旋切装置在乳腺微创外科中的临床应用价值。方法超声实时监控引导下,利用FNCOR真空旋切装置对经术前检查诊断为良性肿物的75例共97个乳腺病灶进行旋切治疗,术后常规送病理检查,并进行超声随访,对比、分析手术区超声图像与术前超声图像,观察术后情况。结果 97个乳腺病灶均被ENCOR真空旋切装置完整切除,直径为0.3～3.2 cm,其中28个为临床不可触及肿块,占28.9%。快速完整切割病灶旋切次数为14刀(3～65刀),用时17 min(2～40 min)术后1d触诊复查,4例在病灶处或切口处出现较为明显的血肿;术后6个月行超声复诊,血肿均消失。2个病灶可见原病灶处低回声,考虑肉芽组织形成为主,建议定期复诊,视情况进一步处置;其余91个病灶手术部位无残留病灶。6个月后复诊时观察伤口愈合情况,1例为瘢痕体质,2个切口可见较为明显的瘢痕,隆起于皮肤表面;73例皮肤切口瘢痕已不清晰。结论超声引导下利用ENCOR真空旋切装置可以准确定位及切除乳房肿物,是一种早期治疗诊断乳腺肿瘤安全、有效、合理、美观、微创、并发症少的方法,值得推广和应用。Objective To study the application of uhrasound - guided ENCOR vacuum biopsy device in minimally invasive breast surgery. Methods Under the screening of dynamic ultrasound, all the 97 breast masses in 75 patients were treated with ENCOR vacuum biopsy device and were analyzed by rnutine pathological examination. All the patients were examined during follow - up visits. Results The diameter of breast masses ranged from 0. 3 to 3.2 cm. 28. 9% of the breast masses （28/97） could not be palpated during the clinical examination . The times of cutting the breast masses averaged 14 （3 to 65 times）. The mean operation time was 17 minutes （2 to 40 minutes）. Small hematoma was seen in 4 patients at D2 after operation but disappeared in 6 mouths during the follow - up examination. Two patients were demonstrated to have the local hypoechogenic area in the local area and suspected of having formed of granulation tissue; who were advised to receive regular follow - up examinations. At surgical sites of the remaining 91 breast masses no residual lesion was found. During the follow - up examination of wound healing after 6 months, one patient proved to be scar constitution, two patients had obvious scars that were intumescent on the skin surface and 60 patients had unclear incision scars. Conclusion Ultrasound - guided ENCOR vacuum biopsy device can accurately position the tumor and resect in quickly. It can be used for early diagnosis and treatment of breast tumor, which is safe, effective, minimally invasive, with few complications . It is worthy of wide application.
