摘 要:目的:应用应变及应变率成像技术研究肺动脉高压(PH)患者右房功能的改变,探索新的评价右房功能的指标。方法:正常对照组33例,PH组50例。分别测量并计算右房被动排空分数(RAPEF)、右房主动排空容积指数(RAAEVI)、右房主动排空分数(RAAEF)、右房射血力指数(RASFI)和右房每博量指数(RASVI)。获取所有入选者右房侧壁应变及应变率曲线,测量右房应变峰值(ASS)、舒张早期应变率E峰峰值(ESR)、房缩期应变率A峰峰值(ASR)和收缩期应变率S峰峰值(SSR)。比较2组之间各参数差别并分析右房功能参数与肺动脉收缩压(PASP)的相关性。结果:与对照组比较,PH组右房管道功能参数RAPEF和ESR降低,右房收缩功能参数RAAEVI、RAAEF、RASFI、ASS和ASR升高,右房储器功能参数RASVI和SSR升高(P均<0.05)。右房功能参数均与PASP有相关性,其中RAPEF、ASS、ASR和SSR相关性较好(r值分别为-0.65、0.74、0.67、0.75,P均<0.01)。结论:PH患者右房管道功能减低,收缩功能及储器功能增强;应变及应变率参数可成为评价右房功能简便的新指标。Objective: To explore the right atrial(RA) functional changes in patients with pulmonary hypertension(PH) by strain and strain rate imaging and find new parameters on right atrial function.Methods: The study included 33 healthy participants as the control group and 50 patients with PH as the case group.The following parameters were measured and calculated: RA passive emptying fraction(RAPEF),RA active emptying volume index(RAAEVI),RA active emptying fraction(RAAEF),RA systolic force index(RASFI)and RA stroke volume index(RASVI).Atrial systolic strain(ASS),early diastolic strain rate(ESR),late diastolic strain rate(ASR) and systolic strain rate(SSR) of RA free wall were measured.The parameters of patients with PH and controls were compared.The correlations of the parameters of RA function and PASP were analysed.Results:In patients with PH RAPEF and ESR were lower;RAAEVI,RAAEF,RASFI,ASS and ASR were higher;RASVI and SSR were higher(P0.05).All the parameters of right atrial function were correlated with PASP.RAPEF,ASS,ASR and SSR had better correlations(r=-0.65,0.74,0.67,0.75,P0.01).Conclusion: The results of the present study indicate that PH is associated with decreased RA conduit function,increased booster pump function and RA reservoir function.Strian and strain rate imaging may be used as a new method to quantify the RA function.
