摘 要:目的探讨妊娠期糖尿病母婴血清及胎盘中内脂素(visfatin)的表达与胎儿生长发育的关系。方法采用酶联免疫吸附法及放免法分别测定30例妊娠期糖尿病(gestational diabetes mellitus,GDM)及30例正常孕妇母婴血清内脂素及胰岛素水平,同时测定空腹血糖、母婴体重及身高、计算胰岛素抵抗指数(HOMA-IR)及体重指数。采用逆转录-聚合酶链反应(RT-PCR)及免疫组织化学方法,检测两组胎盘内脂素基因mRNA的表达及蛋白的表达水平。结果①GDM组血清visfatin、FBG、FIN、HOMA-IR、BMI水平显著高于对照组(P<0.05)。②GDM组新生儿出生体重、RI、脐血visfatin、脐血血糖、脐血胰岛素及HOMA-IR水平显著高于对照组(P<0.05)。③GDM组胎盘组织visfatin mRNA及蛋白表达水平显著高于对照组(P<0.05)。结论 GDM孕妇血清及胎盘内脂素水平的升高可能参与了GDM的发生发展以及胎儿的宫内发育的调节。Objective To investigate the relationship between intrauterine growth of fetuses and the expression of visfatin in placenta and serum of fetuses and mother with gestational diabetes mellitus(GDM).Methods The levels of serum visfatin,fasting blood glucose(FBG) and fasting insulin(FIN) were measured severally by Enzyme-linked immunosorbent assay(ELISA),glucose oxidase,radioimmunity assay.The insulin resistance index(HOMA-IR) was calculated.The expression of visfatin mRNA and protein were separately determined by reverse transcription polymerase chain reaction(RT-PCR) and western blot from 30 pregnant women with GDM(GDM group) and 30 pregnant women with normal glucose tolerance(control group).Results ①The serum levels of visfatin,FBG,FIN,BMI and HOMA-IR were significantly higher in GDM group than those in the control group(P0.05).②The serum of visfatin,FBG,FIN,RI and HOMA-IR were significant higher in fetus in GDM group than those in the control group(P0.05).③The levels of visfatin mRNA and visfatin protein expression in placenta tissue in GDM group were significant higher than those in the control group(P0.05).Conclusion The increased visfatin in serum and placenta may be involved the development of GDM and regulation of intrauterine growth and development of fetus.
