埃索美拉唑与奥美拉唑对体外循环手术患者胃内pH影响

摘　　要：目的:观察埃索美拉唑与奥美拉唑对体外循环(CPB)心脏手术患者胃内pH变化影响。方法:2010年6～12月229例CPB下行瓣膜成形术和瓣膜置换术患者随机分为两组,奥美拉唑组117例术前1周予奥美拉唑片20 mg,po qd,术中奥美拉唑注射液40 mg iv,术后静注奥美拉唑注射液40 mg,iv qd,共3 d;埃索美拉唑组112例术前1周予埃索美拉唑片40 mg,po qd,术中埃索美拉唑注射液40 mg iv,术后埃索美拉唑注射液40 mg,iv qd,共3 d。分别于术后4h,及术后第1、第2天测胃黏膜pH(pHi)。结果:奥美拉唑组术后4 h、术后第1、第2天pHi分别为(5.81±0.54),(5.98±0.26)和(5.13±0.37);埃索美拉唑组分别为(6.21±0.62),(6.38±0.55)和(6.60±0.37)。两组pHi比较,差异无统计学意义(P>0.05)。结论:埃索美拉唑与奥美拉唑均能有效地控制胃内酸度,预防CPB心脏手术应激性溃疡。Objective：To study the effect of omeprazole and esomeprazole on intragastric pH in patients with cardiopulmonary bypass （CPB）. Method：229 patients collected from June to Dec. 2010 were randomly divided into two groups. 117 patients who suffered from the valvular heart disease were scheduled to undergoing CPB in the valvoplasty and/ or valve replacement surgery, and 112 who suffered coronary heart disease （CHD） were scheduled to undergoing CPB in coronary artery bypass grafting（CABG） surgery. The 117 patients received omeprazole 20mg once daily for a week, 40mg intravenously before CPB and then once daily for 3 days. The 112 patients received esomeprazole 40rag once daily for a week, 40mg intravenously before CPB and then 40rag once daily for 3 days. 4 hours, 1 day and 2 days after the operation their pH of gastric intramucosa （pHi） was measured. Result：The PH of gastric intramucosa（pHi） in omeprazole group 4 hours, 1 day, 2 days after operation were 5.81 ±0.54, 5.98 ±0.26 and 5.13 ±0.37. Arid the PH of gastric intramucosa （pHi） in esomeprazole group 4 hours, 1 day, 2 days after operation were 6.21 ±0.62, 6.38 ± 0.55 and 6.60 ± 0.37. There was no difference in the phi in the two groups. Conclusion：Both esomeprazole and omeprazole could effectively inhibit gastric intramucosa and prevent stress ulcer in heart surgery undergoing CPB.
