摘 要:目的探讨淋巴细胞过氧化物酶体增生激活型受体α(PPARα)的表达水平与冠心病(CHD)的关系。方法选择66例冠状动脉造影明确诊断的冠心病患者,其中稳定型心绞痛(SAP)19例,不稳定型心绞痛(UAP)25例和急性心肌梗死(AMI)22例。另选择冠脉造影正常的23例非冠心病患者作为对照组,以ELISA法测定其淋巴细胞中的PPAR-α水平。结果冠心病患者与非冠心病患者组比较PPAR-α的表达水平明显减低(37.97±12.98,57.53±12.87,P<0.01);不同类型冠心病患者的PPAR-α表达水平分别为:AM(I28.05±8.28)<UAP(38.04±12.70)<SAP(49.37±7.50);以AMI、UAP和SAP分别代表病情的严重程度,相关分析显示PPAR-α的表达水平与AMI、UAP和SAP呈负相关(r=-0.723,P<0.01);SAP组比正常对照组PPAR-α的表达水平轻度下降,但无统计学意义(P>0.05)。结论冠心病患者外周血淋巴细胞中PPAR-α的表达下调,随着CHD病情加重PPAR-α的表达进一步下降;PPAR-α可能参与急性冠脉综合征(ACS)的发生发展,并与斑块的不稳定性有关。Aim To investigate the relationship between the level of peroxisome proliferator -activated receptors--α (PPAR-ct)in peripheral blood lymphocytes and the severity of coronary heart disease(CHD). Methods There 66 cases confirmed CHD cases including 25 unstable angina pectoris (UAP group)cases and 22 cases of acute myocardial infarction (AMI group)cases and 19 stable angina pectoris (SAP group)cases;23 healthy people as control group. The level of lymphocytes PPAR-α was determined with ELISA. Results Expression level of PPARα in peripheral blood lymphocytes was significantly lower in coronary heart disease than in controls (57.53±12.87 vs 37.97±12.98,P〈0.01 ). The expression levels of PPARα by peripheral blood lymphocytes in patients with coronary heart disease were (28.05±8.28)in AMI cases〈 (38.04±12.70)in UAP cases〈( 49.37±7,50)in SAP cases;The expression levels of PPAR-α in AMI cases,UAP cases and SAP cases were negatively correlated with the severity of the diseasae (r=-0.723 ,P=0.000〈0.01 );Compared to the control group,the expression level of PPAR-α decreased slighdy,without statistically significant difference (P〉 0.05)0 Conclusions The the expression of PPAR-a in peripheral blood lymphocytes in patients with coronary heart disease was downregulated,the expression of PPAR-α was further decreased along with the exacerbations of the disease, suggesting that PPAR-α may involve in the occurrence and development of acute coronary syndrome and with the instability of the plaque.
