基于专家问卷调查的慢性阻塞性肺疾病常见中医证候要素的研究

摘　　要：目的:运用专家问卷调查的方法提取慢性阻塞性肺疾病常见中医证候要素。方法:在既往工作的基础上,制订专家问卷,对全国8个省市20家医院和科研院所的相关专家开展问卷调查,建立Epidata数据库,通过分析专家对该疾病证候要素常见程度的变异系数、均数、累积百分比,提取慢性阻塞性肺疾病常见中医证候要素。结果:本次调查共发放专家问卷89份,回收率为100%,有效率为100%;根据证候要素常见程度评价情况,将累积百分比≥70%且变异系数≤0.40者作为慢性阻塞性肺疾病的常见中医证候要素。其中病位类证候要素属脏者为肺、肾、脾、心,属腑者为大肠;病性类证候要素属虚者为气虚、阴虚、阳虚,属实者为痰、湿(浊)、饮、血瘀、外寒、热(火)。结论:通过本研究进一步明确了慢性阻塞性肺疾病的常见中医证候要素,为进一步建立该病基于证候要素的辨证标准提供了依据。Objective： To extract the common TCM（TCM） syndrome elements of chronic obstructive pulmonary disease（COPD） with the method of experts＇ questionnaires.Methods： Based on the previous works,we formulated the questionnaire,conducted a questionnaire survey to 8 provinces and municipalities across the country in 21 specialist hospitals and research institutes,established the Epidata database,surveyed the coefficient of variation（CV）,the degree of the mean,the cumulative percent of the COPD syndrome elements,extracted the common syndrome elements.Results： The survey distributed questionnaires were 89 in all,and the recovery rate was 100%,the effective rate was 100%.According to the evaluation of the degree of syndrome elements,a syndrome factor which cumulative percent≥70% and CV≤0.40 can be identified as a common syndrome factor of TCM.Lung,kidney,spleen,heart and large intestine were the common TCM syndrome elements of location of disease.In the common TCM syndrome elements of nature of disease,qi deficiency,yin deficiency and yang deficiency were the deficiency ones;phlegm,damp,fluid-retention,blood stasis,cold and heat（fire） were the excess ones.Conclusions： This study determined the common TCM syndrome elements of COPD,which can afford the foundation to further establish the syndrome diagnostic criteria based on the syndrome elements.
