蕲蛇酶联合肝素治疗对兔下腔静脉血栓溶栓效果及出血风险性的实验研究

摘　　要：目的:蕲蛇酶联合肝素治疗对兔下腔静脉血栓、病变血管内皮及出血风险性的实验研究。方法:新西兰大白兔72只建立急性下腔静脉血栓模型后随机分为肝素组(A组)、尿激酶联合肝素组(B组)、蕲蛇酶联合肝素组(C组),每组24只。各组在血栓形成3 d后给药,分别在用药后的第3、7、10天,通过静脉彩超及病变血管内皮细胞损伤分级评估其溶栓的效果;检测各组血液指标,评估其出血的风险性。结果:在不同的时相,B组和C组的溶栓效果较A组效果好,具有显著性差异(P<0.01),B组和C组溶栓效果均较好,但两组间溶栓效果无差异性(P>0.05)。B组和C组内皮细胞损伤程度均较A组轻,有显著性差异(P<0.01),但B组和C组的内皮细胞损伤程度无差异性(P>0.05)。B组和C组较A组PT值延长、FBG值降低,具有显著差异性(P<0.01),B组较C组的PT值延长、FBG值降低,2组间具有差异性(P<0.05);结论:蕲蛇酶联合肝素能够有效溶解兔腔静脉血栓,减轻内皮损伤,具有良好的治疗及预后效果,并且出血的风险性相对较小。Objective：Experimental study of acutobin combined with heparin to rabbit inferior vena cava thrombosis,endothelial lesions and the risk of blood.Methods：The inferior vena cava thrombus model was established in 72 rabbits and they were randomly divided into three groups：heparin group（A）,group for combination of urokinase and heparin（B）,group for combination of acutobin and heparin（C）,each group including 24 rabbits.Drugs were administrated 3 days after thrombosis.Doppler ultrasound and endothelial injury scale of Vascular lesions were used to assess thrombolytic effect.Coagulation indexes were tested to assess the risk of blood,3,7,10 days after administration.Results：At different phase,the thrombolytic effect of B group and C group were better than A group,having significant difference（P〈0.01）.The thrombolytic effect of B group and C group were better,but it was no difference between two groups（P〉0.05）.The endothelial cell injury level of B group and C group were lighter than A group,having significant difference（P〈0.01）,but it was no difference between two B group and C group（P〉0.05）.B group and C group compared with A group that the value of PT prolonged,FBG values decreased,with significant differences（P〈0.01）.B group compared with C group that the value of PT prolonged,FBG values decreased,with differences between B group and C group（P〈0.05）.Conclusion：Acutobin combined with heparin could effectively dissolve rabbit vena cava thrombosis,reduce endothelial injury,had a good effect of treatment and prognosis,with relatively small risk of blood.
