关节清理术后注射透明质酸钠治疗晚期大骨节病疗效

摘　　要：目的探讨关节清理术与术后关节腔内注射透明质酸钠治疗大骨节病膝的疗效。方法 2004~2010年KBD膝关节病变患者754例882膝,行膝关节清理术,术后按是否使用透明质酸钠分成两组,比较疗效及不良反应,随访期限0.5~1.5 a。通过Lequesne指数评分对治疗前后的膝关节功能进行综合评价。结果两组治疗后Lequesne指数总评分均明显下降,与治疗前比较有统计学意义（P〈0.05）,A组Lequesne评分明显低于B组,组间比较差异有统计学意义（P〈0.01）;关节清理术组（B组）优良率为84.6%,而关节清理术加腔内注射透明质酸钠（A组）优良率达96.9%。结论关节清理术后关节腔注射透明质酸钠是治疗大骨节病膝的一种理想治疗方法。Objective To evaluate the clinical effects of debridement and post operation intra-articulus sodium hyaluronate injection for treating knee Kaschin-Beck disease.Methods 754 case of patients with 882 knee joint osteoarthritic were treated by debridement from June 2004 to June 2010.These patients were divided into two groups of sodium hyaluronate group（group A） and control group（group B）.The curative effect and side effect were studied during the follow-up 0.5 to 1.5 years.The function of the knee was evaluated before and after treatment by Lequesne algo-functional index.Results Lequesne algo-functional index reduced significantly after treatment in two groups and compared with group B Lequesne algo-functional index of group A reduced significantly（P0.01）.The excellent and good rate of group A were 96.9% and group B were 84.6%.Conclusion The satisfactory effect may be achieved in treating knee joint osteoarthritis after debridement with sodium hyaluronate.
