桡骨远端骨折术后早期影像学表现与远期功能的ROC曲线法相关性分析

摘　　要：目的：应用ROC曲线法研究桡骨远端骨折术后早期影像学评分与远期功能的关系.方法：回顾性研究内固定术治疗的桡骨远端骨折的成年患者术后早期Batra改良的Jakim影像学评分及远期腕关节功能的改良Garland and Werley评分的关系,用SPSS 15.0软件进行统计学分析,采取ROC曲线法,以P＜0.05为有统计学意义.结果：共109例患者,平均年龄59.1岁（18～87岁）,改良的Garland and Werley评分平均1.6分（0～12分）,其中优82例,良25例,可2例,Batra改良的Jakim影像学评分平均92分（30～100分）.ROC曲线分析整组资料,60岁以下年龄组和内固定取出组患者影像与功能相关性有统计学意义（AUC分别为0.652,0.785,0.762）.结论：桡骨远端骨折患者内固定术后早期影像学评分与远期功能间存在相关性,对整体而言相关性不高,但对60岁以下患者和内固定取出的患者,两者具有较高的相关性,其术后早期影像学评分对于患者远期功能有一定的预测意义.Objective：To investigate the relationship between early radiographic score and long -term function outcomes in postoperative patients of distal radius fracture internal fixation. Methods：The relationship between early radiographic score and function outcomes in postoperative patients of distal radius fracture internal fixation was retrospectively studied with Batra score, improved Garland and Werley score by means of ROC curve. The analysis was conducted with SPSS 15.0, P 〈 0.05 was for statistic significance. Results： There were 109 cases with the average of 59.1 years; The average Batra score was 92 （30 - 100） and the improved Garland and Werley score was 1.6（0 - 12）. ROC curve showed there was correlation between the patients who were below 60 years and the internal fixation removal group. AUC was 0.652, 0. 785 and 0. 762 respectively. Conclusions：There is correlation in statistics between early radiographic score and function outcomes in postoperative patients of distal radius fracture internal fixation, the correlation is high in the group of patients who are below 60 years. Their early radiographic score can be used as prediction for long - term functional outcomes in clinical practice.
