摘  要:目的探讨BMSCs联合rhBMP-2/rhVEGF对早期激素性股骨头缺血性坏死的治疗作用。方法选取建立早期激素性股骨头坏死动物模型新西兰大白兔30只,随机分为5组,每组6只。A组:模型对照组;B组:单纯减压;C组:减压后植入BMSCs;D组:减压后植入BMSCs联合rhBMP-2;E组:减压后植入BMSCs联合rhBMP-2/rhVEGF。治疗后4、8周时各组取3只兔采用X线观察股骨头结构变化,病理组织学。结果 E组股骨头结构清晰,骨小梁结构完整,骨质破坏明显修复,空骨陷窝减少,大量新生的血管,其骨陷窝阳性数及与血管数目与各组比较差异有统计学意义(P<0.05)。结论 BM-SCs联合rhBMP-2/rhVEGF治疗早期激素性股骨头坏死,促进作用血管形成,显著改善坏死股骨头的血运,促进骨修复。Objective The Secemd the effect of joint application of recombinant human bone morphogenetic protein-2(rhBMP-2)/recombinant human vascular endothelial growth factor(rhVEGF)and bone marrow mesenchymal stem cells as treatment for early avascular necrosis of femoral head of rabbits.Methods An animal model of 30 adult New Zealand white rabbits was set up,which were divided into 5 groups randomly as follows: group A:no treatment;group B:only decompression;group C:decompression with BMSCs transplantation;group D: decompression with BMSCs and rhBMP-2 transplantation;group E: decompression with BMSCs and rhBMP-2/rhVEGF transplantation.The general information of animals were recorded;after 4 and 8 weeks of operation,3 rabbits of each group were chosen randomly to do X-ray and and femoral heads were harvested to do histopathological examination.Results In group E,X-ray film revealed that BMSCs and rhBMP-2/rhVEGF could evidently enhance repairing of the femoral head,where significant new vessel formation was shown,the number of empty lacunae decreased and density of bone trabecula were similar to the normal.The positive ratio of empty lacunae and the angiogenesis showing significant differences compared with other groups(P0.05).Conclusion Joint application of BMSCs and rhBMP-2/rhVEGF can augment angiogenesis of bone tissue and improve microcirculation of necrotic femoral head.
