摘 要:目的:探讨慢性前列腺炎患者感染病原体的分布及耐药率。方法:对456例慢性前列腺炎患者的前列腺液进行病原体检测和药敏试验。结果:456例慢性前列腺炎患者中,256例细菌培养阳性。革兰阳性菌212株,主要为葡萄球菌。革兰阴性杆菌51株,主要为大肠埃希菌和肺炎克雷伯菌,检测出产ESBLs株10株。支原体61例,沙眼衣原体25例。所有细菌未发现对万古霉素耐药菌株,对复方新诺明均较敏感。支原体对交沙霉素、美满霉素和强力霉素三种抗生素耐药率均较低。结论:慢性前列腺炎细菌感染为主,支原体和衣原体感染也有一定比例。不同病原体对不同抗生素耐药率差异较大,适时监测慢性前列腺炎患者感染病原体的分布及耐药率是临床诊断和合理用药的基础。Objective:To investigate the distribution and the antimicrobial resistance of pathogens from the prostatic secretion from chronic prostatitis.Methods: The pathogens obtained were isolated and identified,drug sensitivity tests of the major pathogens from 456 patients were performed.Results: Bacteria were found in 256 pateints out of 456 ones,there were 212 strains of gram-positive coccus,most of which were Staphylococcus;there were 51 gram-negative bacillus,most of which were E.coil and K.pneumoniae including 10 ESBL-producing strains.61 mycoplasma and 25 Chlamydia trachomati were detected in 456 specimens of EPS.All strains were snesitive to vacomycin and cotrimoxazole.There was no resistant strain.Drug resistance of mycoplasmas against jasamycin,minocycline and doxycycline were low.Conclusion: The major pathogenic bacteria in EPS from chronic prostatitis is gram-positive cocci.mycoplasma and Chlamydia trachomatis were also important pathogens.Drug sensitivity tests showed that resistance of pathogens againt different drugs varied a lot.In addition,it is of significant importance to apply bacterial culture and sensitivity test in EPS in the diagnosis and treatment of chronic prostatitis.
