2型糖尿病血浆胰岛素水平与颈总动脉内-中膜厚度的动态观察

摘　　要：目的探讨血浆胰岛素水平与动脉粥样硬化的关系。方法动态观察2型糖尿病患者血浆胰岛素水平与颈总动脉内-中膜厚度(intima-mediathickness,IMT)。将研究对象按治疗方法分为A、B两组,A组为注射胰岛素组,30例;B组为口服降糖药组,35例。观察两组患者空腹血浆胰岛素(FINS)、餐后2小时血浆胰岛素(2HINS)、体重、IMT及有、无动脉粥样硬化斑块等项目,随访5年,每年进行一次上述项目的检测。结果研究前:A组FINS、2HINS较B组高,差异有统计学意义,P<0.05;A、B两组体重、IMT增厚及动脉粥样斑数比较,差异无统计学意义,P>0.05。研究后:A组FINS、2HINS、体重较B组高,差异有统计学意义,P<0.05;A、B两组IMT增厚及动脉粥样斑数比较,差异无统计学意义,P>0.05。结论注射胰岛素的2型糖尿病患者存在高胰岛素血症及体重增加,而未显示动脉粥样硬化的增加。Objective To approach the relationship of artherosclerosis and the level of plasmic insulin.Methods The level of plasmic insulin and the common carotid artery intima-mediaethickness（IMT） were dynamicly observed in Type 2 diabetes.Sixty five patients were enrolled,they were divided into two groups.One（A group） was treated with injecting insulin（n=30）,the other（B group） was treated with oral hypoglycemic agents（n=35）.The level of plasmic fasting insulin（FINS）,the level of plasmic posting 2 hours insulin（2HINS）,the common carotid artery IMT when end-diastol,whether plaque had formed or not,and weight were detected in all patients in every year.All patients were followed up for 5 years.Results Before study the level of FINS and 2HINS in A group were higher than that in B group（P0.05）,The differences of the weight,IMT,the number of plaque between A group and B group were non-significant（P0.05）.After study the level of FINS,2HINS,weight in A group were higher than that in B group（P0.05）,The differences of the IMT and the number of plaque between A group and B group were non-significant（P0.05）.Conclusion Hyperinsulinism and weight gain exist in type 2 diabetic patients who treated by injecting insulin,but there is no increased artherosclerosis in those patients.
