类风湿性关节炎患者免疫功能的观察

摘　　要：本文对43例类风湿性关节炎(RA)患者与53例健康人对比进行了比较系统的免疫学检测,其结果提示RA患者的淋巴细胞转化率与E花环形成率显著低于健康人,前法分别为0.50±0.07与0.64±0.10,P<0.001,后者分别为0.48±0.11与0.61±0.11,P<0.001,两者均具有极度显著性差别;血清IgG与IgA显著高于健康人,IgG分别为2902±1304与1416±341g/L,P<0.001,IgA分别为2300±1710与1380±570mg/L,P<0.001,两者均具有极度显著性差别,IgM与健康人对比亦呈现升高,其分别为1470±900与1320±500mg/L,但经统计学处理,P>0.05,无显著性差别;血清总补体(STC)水平显著低于健康人其分别为7463±2083与9363±2076ku/L,P<0.001,有极度显著性差别;循环免疫复合物(CIC)经聚乙二醇(PEG)沉淀法测定结果,阳性率为58.14%,经抗补体法测定结果,阳性率为30.23%,而以上两种方法对50例健康人测定结果,则均为阴性;自身抗体测定结果,抗核抗体(ANA)阳性率为30.23%,平滑肌抗体(SMA)阳性率为32.56%,血清类风湿因子(RF)阳性率为67.44%。In this paper, we have made comparatively systemic immunological determination on 43 patients with rheumatoid arthritis (RA) in contrast with 53 health persons. The result showed that the lymphocyte transforming percentage and the E rosette forming percentage of RA were obviously lower than that of the healthy ones, The former were 0.50±0.07 and 0.64±0.10. P<0.001 respectively. The latter were 0.48±0.11 and 0.61±0.11, P<0.001, Both were all extremely obvious difference; The serum IgG and IgA were obviously higher than those of the healthy ones. The IgG was 29.02±13.04 and 14.16±3.41g/L respectively P<0.001, The IgA was 2300±1710 and 1380±570mg/L respectively P<0.001, Both were all extremely obvious difference, The IgM of patients with rheumatoid arthritis in contrast with the healthy persons was also higher, They were 1470±900 and 1320±500mg/L separately But they were handeled by statistical method, P>0.05, and no obvious difference. The level of the total complement(STC)in serum was obviously lower than that in healthy ones, They were 7465±20.83 and 9363±20.76Ku/L respectively, P<0.001, They were extremely notable difference; The result of the circulatory immune complex (CIC) was detected by polyethylene glycol (PEG) precipitation method. The positive rate was 58.14%, The result of determination by anticomplement method is that the positive rate was 30.23%, But the result of determination by the above two methods on 50 health persons was all negative; The result of determination in autoantibodies is that the positive rate of antinucler (ANA) was 30.23%. The positive rate of smooth muscle (SMA) was 32.56%, The positive rate of serum rheumatoid factor (RF) was 67.44%.
