体外循环状态下肺野旷置白细胞与氧衍生自由基

摘  要:为了探讨体外循环状态下肺野旷置白细胞与氧衍生自由基产生的关系,在体外循环初、末分别采取动脉血测定血清超氧化物歧化酶的活性(μ/mg pro)和血清丙二醛含量(nmol/ml)。在体外循环结束后,同时采取右室、左房血进行白细胞计数和白细胞分类计数,用中性白细胞差值反映肺循环白细胞旷置。结果显示:体外循环后白细胞总数明显增高(P<0.01),其中以中性白细胞及杆状核增高为主(P<0.01),左房与右室白细胞差值为2.018×10~9/L,证实肺内旷置。体外循环末血清超氧化物歧化酶活性明显低于体外循环初(P<0.05),而体外循环末血清丙二醛含量明显高于体外循环初(P<0.05)。因此认为,肺内截获白细胞与氧自由基产生及脂质过氧化速率有关。In order to determine the relationship between pulmonary leukosequestration and oxygen free radical generation, We prospectively studied 20 patients undergoing cardiopulmonary bypass for open-heart procedure of congenital heart discases. Serum levels of SOD and MDA were measured at the begining and the end of extracorporeal circulation cell counts and differential counts were made from blood in the RV and LA after CPB. The results showed that CPB caused significant increase in WBC, Polymorphonuclear cells were the main species of leukocytes sequestered in the lungs. (Median cell difference 2.018×10~9/L, P<0.05 )Serum level of SOD was reduced and MDA was increased during CPB (P<0.05). So we consider that both pulmonary leukosequestration and excess oxygen free radical generation contribute to the pathophysiologic changes of extracorporeal circulation.
