摘 要:目的:探讨G raves'病和桥本氏甲减患者外周血中Th1、Th2、Th17细胞和CD4+CD25+FOXP3+调节性T细胞(Treg)的比例及其意义。方法:选择新诊断但未治疗的G raves'病(GD)患者25例、桥本氏甲减25例和正常对照者25例,通过流式细胞技术测定其外周血中Th1、Th2、Th17和CD4+CD25+FOXP3+Treg细胞占CD4+T细胞的比例。由于Th1、Th2、Th17细胞分别分泌IFNγ-、IL-4、IL-17细胞因子,所以在本文中Th1细胞表型是CD4+IFN-γ+IL-4-IL-17-;Th2细胞表型是CD4+IFN-γ-IL-4+IL-17-;Th17细胞表型是CD4+IFN-γ-IL-4-IL-17+。结果:GD组与桥本氏甲减组外周血Th1、Th2、Th17细胞比例均高于正常对照组(P<0.05),而两者的CD4+CD25+FOXP3+Treg细胞比例与对照组相比无明显差异(P>0.05)。结论:在新诊的GD和桥本氏甲减患者中外周血Th1、Th2、Th17细胞水平都升高,两者间的类似现象可能在甲状腺自身免疫病的发病机制中有重要意义。Objective To investigate the proportions and significance of Th1,Th2,Th17 and CD4+CD25+FOXP3+ regulatory T (Treg) cells in peripheral blood in patients with Grave's Disease(GD) and Hashimoto's Thyroiditis with hypothyroidism.Methods 25 patients with Grave's disease(GD group),25 with Hashimoto's Thyroiditis who developed hypothyroidism(HT group),and 25 healthy subjects(control group) were selected,all the patients newly diagnosed and untreated.The proportion of Th1,Th2,Th17 cells and CD4+CD25+FOXP3+ Treg cells within CD4+ T cells were evaluated by technique of flow cytometry.By the definitions in this study the phenotype of the Th1 cells were CD4+IFN-γ+IL-4-IL-17-,the phenotype of the Th2 cells were CD4+IFN-γ-IL-4+IL-17-,and CD4+IFN-γ-IL-4-IL-17+ were the phenotype of the Th17 cells.Results The proportions of peripheral Th1,Th2,Th17 cells were higher in both GD and HT groups than those in control group(P0.05).However the proportions of CD4+CD25+FOXP3+ Treg cells were not significantly different among each examined group(P0.05).Conclusion We found the similar phenomena that enhanced levels of Th1,Th2,Th17 lymphocytes in the peripheral blood in newly diagnosed patients with GD and Hashimoto's Thyroiditis with hypothyroidism may play an important role in the pathogenesis of thyroid autoimmune diseases.
