摘 要:目的:研究我国北方汉族人群PPARγ2基因Pro12A la多态性与2型糖尿病肾病(DM2-N)易患性的关系。方法:应用聚合酶链式反应-限制性片段长度多态性(PCR-RFLP)方法检测了78例2型糖尿病(DM2)患者(糖尿病肾病组患者41例,非糖尿病肾病组患者37例)及38例正常对照组的PPARγ2基因Pro12A la多态性;应用酶联免疫吸附试验(ELISA)测定血清TNF-α(肿瘤坏死因子-α)水平。结果:①在中国北方汉族人中糖尿病肾病(DN)组基因型分布与非DN组及正常对照组比较无显著性差异(2χ=0.97,P>0.05;2χ=0.90,P>0.05)。DN组等位基因频率分布与非DN组及正常对照组比较无显著性差异(2χ=0.92,P>0.05;2χ=0.86,P>0.05)。②DN组患者血清TNF-α水平显著高于非DN组及正常对照组(P<0.001),非DN组糖尿病患者血清TNF-α水平显著高于正常对照组(P<0.01)。结论:PPARγ2基因Pro12A la多态性与中国北方汉族人DM2-N易患性之间无明显相关。TNF-α在DM2及DN发病过程中可能起到重要作用。Objective To study the relationship between peroxisome proliferator activated receptor γ2 gene polymorphism and susceptibility of diabetic nephropathy in type 2 diabetes mellitus.Methods Polymerase chain reaction-restriction fragment length polymorphism(PCR-RFLP) was used to examine the polymorphism of peroxisome proliferator-activated receptor γ2 gene in 78 patients with type 2 diabetes mellitus [41 cases with diabetic nephropathy(DN group),37 cases without diabetic nephropathy(non-DN group)] and 38 controls.The serum TNF-α was measured by ELISA.Results The frequency of Pro allele and Ala allele are 0.957 and 0.043 in Han people of North China.There was no significant difference in the frequency of genotypes(PP,PA,AA) among DN group,non-DN group and control group(χ2=1.13,P0.05).There was no significant difference in allele freguency(Pro,Ala) among DN group,non-DN group and control group(χ2=1.08,P0.05).Compared with non-DN group patients and controls,serum TNF-α level of DN group patients was significantly higher(P0.001).Serum TNF-α level of diabetic patients of non-DN group was apparently higher than that of controls(P0.01).Conclusion There was no significant association between PPARγ2 gene polymorphism and susceptibility of diabetic nephropathy in type 2 diabetes mellitus in Han people of North China.But serum TNF-α level of patients with diabetic nephropathy was obviously higher than those of controls and patients without diabetic nephropathy.Serum TNF-α level of diabetic patients was higher than that of controls.
